Job summary

The ChatHealth 0-5-year-old text messaging service supports parents and carers with expert advice from health visitors and community nursery nurses relating to a wide range of children's health and wellbeing issues including sleep, feeding, crying babies, development and behaviour. As well as giving advice, the team signpost to appropriate online resources, face-to-face services, and a range of support. ChatHealth is an integral part of the innovative and continuous digital development of the 0-19 Public Health Nursing Service offer to children, young people and families across Hampshire.

Due to the continuing success of 0-5 ChatHealth, Children and Family Services is developing this as a stand-alone service with its own dedicated team. We need experts committed to delivering quality care through this innovative digital approach which supports improved access for our families.

The service runs Monday to Friday 9-5.

This new service will be an opportunity for you to work more flexibly across Monday-Friday by working from home and making use of Children and Family Services bases across Hampshire. We will ensure you will be able to meet regularly as a team to receive great support and leadership from the wider health visiting service. Safeguarding, clinical and management supervision will be provided, and you will have access to a wide range of development and learning opportunities to ensure you maintain and grow your professional skills and knowledge.

Main duties of the job

The primary role of the Community Nursery Nurse is to deliver elements of the Healthy Child Programme including the 6 High Impact Areas as delegated by 0-5 ChatHealth Team. The Community Nursery Nurse should ensure the quality of service delivery to children, young people and their families is aligned with the commissioned requirements of the Healthy Child Programme 0-19yrs and the Trust vision and values. This includes working in partnership with internal and external colleagues and partners.

*To work within the 0-5 ChatHealth Service to deliver delegated components of the 0-19 Healthy Child Programme (HCP), (DH 2009), monitoring quality and compliance and reporting risks by exception to the 0-5 ChatHealth Health Visitor

*To contribute to the delivery of the HCP and 6 High Impact areas via the 0-5 ChatHealth Service, including giving advice and support on;

Transition to Parenthood and the Early Weeks including infant attachment

Maternal Mental Health (Perinatal Depression)

Breastfeeding (Initiation and Duration)

Healthy Weight, Healthy Nutrition (to include Physical Activity)

Managing Minor Illness and Reducing Accidents (Reducing Hospital Attendance/ Admissions) including the prevention of accidents and promotion of key messages around child safety. Support the management of childhood illnesses through health promotion

Health, Wellbeing and Development of the Child Age 2 -- Two year old review (integrated review) and support to be 'ready for school'. To work in partnership with families and agencies to support the child's smooth transition into school.

*To work within Hampshire Safeguarding Children's Partnership and the Trust Safeguarding Policy and Procedures

*To contribute to interventions identified by the Health Visitor and refer children and families where there are identified vulnerabilities to appropriate support and services

*To deliver a safe and effective service using service user feedback to improve patient experience and outcomes

*To signpost to programmes of care for children 0-5 years with complex health needs

*Escalating IT issues and network outage to the CDTL as a matter of urgency

*To support Health Visitors with integrated working; developing and maintaining relationships with statutory and non-statutory agencies
