Job summary

We are currently recruiting for a Physiotherapist to be part of the Havant and Waterlooville Integrated Care Team, based at Havant Health Centre. You will be joining a friendly and supportive team within a leading organization that is both forward thinking and innovative. The post would suit a current band 6 Physiotherapist or a current band 5 looking for development into a band 6 position.

This Physiotherapist position is designed to contribute to avoiding hospital admission and rehabilitating patients in their own homes throughout the Havant, Hayling and Waterlooville localities. Clients accessing this service will have a wide range of pathologies and will be predominantly (but not exclusively) in the elderly care group. This post offers the opportunity to work closely with all members of the MDT both in primary, secondary care, adult Services as well as with care/nursing home staff.

Following a period of induction and mentorship you will be responsible for coordinating a plan of care and rehabilitation, through identifying person-centred rehabilitation goals following assessment, working within their your scope of practice autonomously as a registered practitioner. You will need excellent communication skills and be competent in linking with the larger multidisciplinary team.

Main duties of the job

To provide advanced clinical and professional Physiotherapy to the Community Therapy Team, and to provide support to the Integrated Community Care team leaders in ensuring that the team contributes to the implementation of Southern Health Clinical Governance Strategies.

Undertake Physiotherapy assessments of patients with diverse and complex conditions, providing a clinical diagnosis (where appropriate), development, and delivery of individualised treatment programmes.

To support the case management process by providing expert clinical advice and therapy to patients with a variety of complex conditions living in the community who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission to secondary care

As a supervisor, delegate tasks to assistant practitioners and junior therapists relevant to their skill level and competence
