Set on the outskirts of the New Forest, Ford Ward is based at Fordingbridge Hospital so is an idyllic location with easy access to the New Forest.

Ford Ward is a 15 bedded unit and has a large day room, dining room and therapy room which are all conducive to rehabilitation. We are reintroducing group activities to support the best recovery for our patients. The Community beds service aims to rehabilitate or re-able people to return to their own homes within a few weeks of their admission/transfer.

We are looking for an experienced, dynamic, compassionate and dedicated OT to join our Rehab Team across 7 days. You will be responsible for day to day leadership of the physical health team and provide clinical support and supervision to your team. The successful applicant will work as part of an inpatient nursing/therapy team involved in Patient's rehab to enable them to achieve their individual goals in order to be discharged safely from hospital, and you must have excellent communication skills in order to work effectively with both the integrated team and wider community care provider.

Main duties of the job

*To provide specialist therapy skills to the therapy team within the inpatient setting.

*To assess and treat own caseload, which may include complex presentations and to maintain own records as an autonomous practitioner.

*To assist senior staff in the education and supervision of junior colleagues, assistants and students.

*To assist senior staff in daily management of the team and provision of therapy service, deputising for the team leader in their absence.

*To assist senior staff in development and maintenance of an efficient and effective therapy service.

*To contribute to the inter-professional working within the Inpatient therapy team and Locality by teaching, learning & practicing inter-disciplinary skills & techniques as defined by an agreed competency framework or training scheme.

Job responsibilities

*To provide holistic specialist assessment of patients, using analytical and judgment skills.

*To provide appropriate patient centred treatment using evidence based practice where-ever possible. Patients will present with acute or chronic conditions and complex multi-system pathologies e.g. neuro, respiratory conditions, orthopaedic rehabilitation and age related deterioration.

*To devise effective care plans for each patient, recognizing him or her as an individual. The plan of care, which has been developed in conjunction with the patient, carer and relevant others, should be outcome based and ensure appropriate pathways of care and communication via liaison and referral to other agencies as required.

*The goals and objectives of any intervention are clearly established and negotiated, and where appropriate can be assessed through use of outcome measures/ objective markers.

*To provide the treatment programme or where appropriate direct the intervention as necessary through Rehabilitation Assistants, or other members of the multi-disciplinary team.

*To provide a regular system of monitoring patients, whose treatment has been delegated to a Rehabilitation Assistant.

*To advise, support and empower (both spontaneously and planned) patients and their families/ carers in order to promote understanding of their condition, aims of therapy and aid self-management where appropriate.

*To communicate verbally and non-verbally with patients and carers in spontaneous and planned situations. To utilise motivation, persuasive and reassurance skills to communicate diagnosis, gain consent and understanding of therapy treatment programs.

*To communicate effectively to overcome frequent barriers to communication due to patients with complex emotional, physical and psychological barriers, e.g. confusional states, expressive and receptive dysphasia, depression, hearing and visual impairment, altered state of consciousness or patients with significant perceptual and cognitive impairments.

*To communicate effectively to provide advice regarding therapeutic management of patients to the MDT and work collaboratively with the MDT to deliver patient-centred services. This includes regular active participation in ward rounds, multi-disciplinary meetings and discharge planning.

*To facilitate safe discharge at the optimum point of care for inpatients, including assessing risk, undertaking home assessments if appropriate, ordering equipment and providing appropriate advice for patients/ carers.

*To ensure appropriate arrangements are made for the transfer of care, including some community follow up (where applicable).

*To provide transfer/discharge reports on the patient's management to relevant agencies as required.

*To maintain accurate and up to date patient records as an autonomous practitioner and in accordance with legal requirements, Trust standards and Professional Standards.

*To be responsible for organising and planning of own caseload to meet service and patient needs with support of other team members. This will include the need for regular prioritisation skills and flexibility to meet the varied and changing demands of the service on a daily basis.

*To work in various ward locations, as agreed with the line manager, as per patient needs and service requirements.

*To submit accurate and timely statistical information on patient activity as required by the service/ Trust.

*To participate in the collection of data, undertake literature searches and reviews etc in support of evidence based clinical audit in the relevant field of interest.

*To maintain excellent clinical practice through the CPD process, and maintain a Professional Portfolio. This may include seeking peer and senior support from senior colleagues working in other areas of the Trust.

*To keep up to date with current knowledge and advances in area of therapy practice, contributing to team knowledge through presentation of a piece of Clinical Governance project work.
