Registered Mental Health Nurse looking for something a bit different?

Across Hampshire and Isle of Wight (HIOW) we have launched our plan to transform experiences in adult mental health in a programme called 'No Wrong Door'. This is a new and innovative service led by the local NHS, councils, Public Health, GPs, voluntary and community partners and people with lived experience of mental illness.

Through our research adults with a serious mental illness have told us first contact is the most important point in successfully starting their recovery. When people with a serious mental illness are at their most anxious, they face too many choices with little support.

To tackle this, we are going to:

1.Ensure that at the start of a person's recovery, wherever and however a person accesses care they are met with compassion, understanding, and supported with both their physical and mental wellbeing.

2.Build teams around communities that are close to people's homes, by working with our partners including the voluntary sector, making sure people's recovery is close to their personal support networks.

3.Provide people who are working across these physical and mental health services with the right support and training, making sure we have creative career options across the area.

Main duties of the job

Southern Health NHS Trust will be working with Primary Care Networks, known as PCNs, to develop teams of health and care professionals including wellbeing practitioners, nurses, experts in psychology and psychiatry, social workers, and education and employment support workers.

Southern Health NHS Trust's Portsmouth and South East Division and both the Swan and East Hampshire PCN's are now in position to offer these exciting opportunities for innovative, flexible, professionals who are able to network and be part of the building blocks for the transformation of frontline mental health services.

Strawberry Health PCN

This PCN consists of two surgeries, Vine Medical Group and Homewell Practice. Based in the Havant and Waterlooville area. We are an innovative and agile organisation committed to team working through a diverse workforce. We are passionate about delivering quality care to our patients but also ensuring our staff are fully supported. Employed by Southern Health NHS Trust and funded jointly these roles will be based within PCNs to develop relationships between organisations and influence the transformation to provide the right services for our population by the right people at the right time.

This is a new role working collaboratively across primary and secondary care, the role will therefore be subject to change as it develops.

To work closely with secondary mental health and primary care and ensure good communication between services.

To champion a culture based on principles of recovery, choice and personalised care,

providing a role model to those who the practitioner works with.

The post holder will also provide some direct clinical care within the PCN. It will provide support to staff within the Primary Care Network and be engaged within the local community mental health team.

*To work with Primary Care stakeholders in South East area to support and help deliver the transformation work around "No wrong door".

*To be able to deliver therapeutic interventions to service users whose immediate needs may need help in a primary care setting.

*To support the planning and coordination of service users' care in primary care, including those on the SMI list liaising with teams and agencies as required. To work on signposting to appropriate agencies for ongoing support.

*To discuss primary care referrals that require referral to secondary mental health care prior to making a referral

*To work closely with the secondary care mental health practitioner to provide a link between primary care services and secondary care for the Mental Health Team.
