Job summary

This is a new, exciting, and innovative post to support the needs of the local population living with mild to moderate mental health difficulties. We are looking for a registered nurse, occupational therapist, or social worker to take a central role in the development of the mental health offer in our Primary Care Network (PCN). Through the delivery of excellent, compassionate services to promote the recovery and well-being of patients, carers and families.

At New Forest PCN we support patients attending Wistaria and Milford, Chawton House, Lyndhurst and New Forest Medical Group Surgeries. We have a population of 36, 228, many over 65, however we see many people of working age in need of support to aid their mental health recovery. At New Forest PCN we can offer flexible working, including job shares, later start and finish times and some Hybrid working.

You will be joining a well-established, friendly, and committed team who prioritise patient centred practice. New Forest PCN are exploring innovative options to meet the patient demand. The main base is at the War Memorial Hospital, Milford on Sea and is in walking distance from the village and beach, which aids work life balance, as there is the opportunity to take breaks in a wonderful setting.

Main duties of the job

As an Advanced Mental Health Practitioner your role will include:-

*Leading the development of the mental health service collaboratively within the PCN and with stakeholders.

*You will work in a multi-agency way both as part of the PCN. Facilitating the interface between Primary Care and Secondary Mental Health Services to provide specialist advice and support, assessment, treatment, education and solution-focused approaches to the local Primary Care Teams.

*Leading by example, being professionally accountable and responsible for patient care.

*Triaging and undertaking initial assessment, signposting to appropriate services, or providing time-limited psycho-social therapeutic interventions directly, in GP surgeries, patient's homes and other primary care settings.

*Providing clinical advice to GPs such as specialist opinion, this will contribute to the safe and effective day to day operation of the PCN and the efficient and timely management of clinical enquiries and decision-making.

*Maintain clinical knowledge and skills by attending training and applying this in practice to support the development of the team.

To provide training to students monitoring and evaluating learning outcomes, providing feedback and assessing competencies to facilitate learning in practice.

Participate in the recruitment and selection of staff in the clinical team.

Act as the lead in Mental Health Clinician strategic forums between the Trust and GP Federations.

Lead practice development initiatives within the team.

Plan own workload and that of the evolving practice-based mental health team, managing competing demands to ensure care is delivered according to clinical priority and complies with defined indicators

  • Ability to evidence Continuing Professional Development since registration.
  • Certificate in mentoring, teaching & assessing in clinical practice as appropriate to own profession.
  • Leadership or management qualification
  • Clinical Supervisor.
  • Recognised sign-off mentor (or equivalent) for students or must be achieved within agreed timescale.
  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent)
  • NMP (or willing to work towards within an agreed timescale)
  • Post-graduate qualification in a relevant area
  • Training or experience of delivering CBT or DBT
