Job summary

We are currently recruiting for full and part time Mental Health trained Registered Mental Health Nurses/Social Workers to join our award winning and friendly team.

The 111 Mental Health Triage Service covers the Hampshire area including Surrey & Borders and the Isle of Wight. The service is employed by Southern Health Foundation Trust and we work closely with the South Central Ambulance Trust. This involves working closely with other Healthcare Professionals including Paramedics & Registered General Nurses.

Working within this role, practitioners provide a holistic mental health traige on the telephone for people that contact 111 with mental health needs.

The service also runs a Mental Health Car which can visit and assess people in their own home and other social environments. This involves a mental health practitioner working alongside a Paramedic who will assess the patient on scene to provide the most suitable intervention.

The service is accessed when patients ring 111, they will firstly be triaged by a call handler that will determine their needs. If the person has mental health needs, they will be placed into the mental health queue whereby a mental health practitioner from our team will check their history, then call the patient back to provide support.

The service supports all ages and can provide a wide range of assistance that can include listening, reassurance, brief psychological interventions as well as referral to primary and secondary services.

Main duties of the job

This role will involve being a member of our award nominated telephone triage team, working closely with colleagues in South Central Ambulance Service as well as other agencies including Primary and Secondary Services. There is also an option to work on the Rapid Response Vehicle which can visit and triage people in the community.

This role will enable you to work with service users in a recovery focussed way, using professional skills in triage and providing suitable interventions.

To work as an autonomous practitioner within a nursing team and provide high quality complex clinical telephone triage using knowledge, skills, critical thinking, and professional judgement supported by clinical software.

To provide advice supported by the clinical triage tool to assess for urgency and priority with the view to refer onto other healthcare professionals if required.

To accurately record all contacts with service users, or arrange transmission of such data, in RiO, System1 and other mental health electronic records in line with the organisational record keeping policy.

Whilst using Patient Management Plans, ensure they are effective and to raise any concerns via demand practitioners and team leaders.

To provide clinical and professional expertise as required to call centre colleagues and other professionals who phone in.

To manage the calls to patients, relatives, and the general public in a calm professional manner. Treating them with dignity and respect at all times.

Be able and confident to identify the needs and take action to adapt methods of communication, for the differing needs of patients and callers, especially in stressful and difficult situations.

To maintain professional standards across the NHS trusts both internally and externally.

To be confident in adapting, escalating and maintaining control when dealing with safeguarding or life-threatening situations under pressure with patients and relatives.

Promptly refer any issues/concerns outside the scope of the post holder's responsibilities to the appropriate team leaders or using the SHFT escalation process.

Work as an effective member of a high-performance team, ensuring self-awareness of surrounding environment and events.

Have knowledge of escalation policies and emergency procedures as necessary.

Reflect the diversity of needs in healthcare communities by utilising all available resources to facilitate access to the service where there are barriers to communication and understanding, e.g. text phone, interpretation services.

To operate a 24-hour service and work shift patterns including weekends, bank holidays and nights.

To provide therapeutic interventions, in line with agreed care plans and risk assessments, in order to prompt the principles of recovery, choice and personalisation, which value and empower the service user.
