community mental health team in County Durham.
This post
is for a 1.0 Specialty Doctor doctor covering 33,549 people over 65 years
in Sedgefield & Dales Community Mental Health Team based at Auckland Park
Hospital in Bishop Auckland, 5 days a week at Sedgefield & Dales
Community Mental Health Team based in Bishop Auckland.
Durham & Tees Valley Mental Health Services for Older
People (MHSOP) provides a comprehensive mental health service to older people
living in the region and works in partnership with acute and community health
services provided by 3 acute Trusts ( County Durham & Darlington NHS
Foundation Trust, North Tees & Hartlepool NHS Foundation Trust and South
Tees Hospitals NHS Foundation Trust)), and with 5 local authorities (Durham County
Council, Darlington Borough Council, Stockton Borough Council, Hartlepool
Borough Council, Middlesbrough Council, and Redcar & Cleveland Borough
Council ). The service serves an over-65 population of 257,007. The service
provides assessment and treatment of organic and functional mental illness in
the population over 65 years old, although younger patients who are felt to be
more appropriate for the service are also accepted. Patients are accepted into
the service on the basis of need rather than age. Referrals are from local
primary care teams and also from colleagues in the acute trust.
Main duties of the job
The
post holder will work along with two consultant psychiatrists and another
specialty doctor at Auckland Park Hospital. The service provides mental health
services to older people with mental health problems and age-related
complexities. We also accept referrals for young onset dementia patients. Each
team has administrative support and co-located social workers. The team uses
the Care Programme Approach (CPA) to ensure regular reviews and risk assessments
are completed, with supervision in place in accordance with trust policy. The
service is commissioned by the North Durham Clinical Commissioning Group and
the Durham Dales, Easington and Sedgefield Commissioning Care Group, and
supported by North East Commissioning Support Service. This vacancy has arisen
following the overseas relocation of the previous post-holder.
Sedgefield and Dales Community
Team provides mental health assessments for people who have memory problems or
who have other mental health problems requiring secondary care intervention in
the locality. The service has memory clinics that assesses and diagnoses people
with dementia. They provide ongoing support to the patient and their carers,
prescribe medication if appropriate and monitor their mental health. The team
accepts people who are predominantly aged 65 and over. However if the patient
is younger but it is felt that the service is best placed to meet their needs,
the team will assess them.

As previously noted, the post
holder will work with the Sedgefield and Dales Community Mental Health Teams
across both Organic and Functional cells as well as the Care Home Liaison
during the working week. The
post holder will work along with two consultant psychiatrists and another
specialty doctor at Auckland Park Hospital.
The community mental health team uses the Care Programme
Approach (CPA) to ensure regular reviews and risk assessments are completed,
with supervision in place in accordance with trust policy. The team work in
line with the principles of New Ways of Working, and so the post holder would
not be normally expected to care coordinate patients, but would be expected to
provide input into cases where psychiatric input would be beneficial to
patients and/or in cases of complexity. Therefore, the specialty doctor is
unlikely to hold a significant personal caseload, other than a small number of
patients for whom they are leading treatment (around 20 patients in total). The
teams operate well-established MDT huddles where clinical cases are discussed.
The specialty doctor would be expected to participate in this. New referrals
are allocated for assessment on the basis of patient need. Whereas, the vast
majority of new patient assessments are undertaken by CPNs, some referrals may
be more appropriate for psychiatrists, and these will be allocated to medical
staff based on need. The post holder will help to run
memory, diagnostic, lithium and other outpatient clinics. Clinical contacts
could take place at the base in a clinic setting, but also at patients homes
and in care homes. The post-holder will also be expected to provide support and
advice to the wider team within their sphere of expertise. They will be
expected to do urgent mental health assessments and it is expected that the
prospective candidate will either be approved under Section 12 of the Mental
Health Act 1983, or will be able to achieve this as soon as possible after
appointment. The Trust would be happy to support the process of gaining this
approval. The post holder is expected to be involved in referring patients for
relevant investigations, and will also treat patients in line with national and
Trust guidelines. Regular supervision will be provided by Dr Tahir Salim in
line with the needs and experience of the post holder.
In terms of indicative community workload, our modelling predicts
that the post holder will hold about 15 - 20 patients on their personal
caseload. They completed approximately 20 initial assessments and approximately
250 further clinical contacts per year. This is the total across the community team.
In practice, over 90% of referrals to the team are normally assessed by nursing
staff, and on average the previous post-holder conducted no more than 1 new patient
assessment/week (and in some weeks had no new patient assessments). The
remainder of appointments are follow-ups or diagnostic appointments. These are
seen either in clinics or as home visits. In general, clinics run between 30 - 60
minutes per patient (depending on clinical need) and so the average clinic
would consist of approximately 2 4 patients. As home visits require travel
time, the post holder would usually only be required to conduct 2 home visits/session.
