This post is for a Consultant Psychiatrist to join a Child & Adolecent Crisis, Liaison and Intensive Home Treatment Team

Main duties of the job

The Crisis, Liaison and Intensive Home Treatment Team are based at Lanchester Road Hospital. This is one service with 2 integrated teams offering a crisis and liaison service and a Home Treatment Team. This team is a community, out-patient specialist team who provide psychiatric input into complex, high risk cases presenting in crisis in the acute hospital setting (A&E or paediatric wards), in community settings and occasionally on a Section 136.

The Intensive Home Treatment team works alongside the Crisis Team, with the remit of providing intensive community based treatment to those young people who present at risk of hospital admission, or who are stepping down from a period of inpatient treatment. This is additional to standard community support which is ongoing to ensure patient continuity.

Job description

Job responsibilities

This post is a 10 programmed activity post which is shared between the Intensive Home Treatment and Crisis Resolution teams at 7.5PA clinical. An additional 2.5PA would be allocated to supporting professional activities.

The new postholder would have the opportunity to be an Educational Supervisor for training grade doctors.

It is expected that the post holder will cross cover with the Specialist Eating Disorders team consultant responsible for EDIHT, thereby ensuring that SPA and annual leave can be facilitated.

The IHT service offers flexible opening hours, each week from 8am-10pm

The CAMHS Crisis team operates 24/7

The post holder will provide psychiatric input into complex, high risk cases presenting in crisis in the acute hospital setting (A&E or paediatric wards), in community settings and occasionally on a Section 136.

Following assessment they will contribute to the formulation, diagnosis and joint management plans for patients presenting in crisis and liaison settings and for those accepted onto the Intensive Home Treatment caseload. As part of the crisis role they will be required to undertake Mental Health Act assessments where this is necessary. They will. provide routine and adhoc specialist medical knowledge and leadership to the crisis team and IHT team and form an integral part of the Multi-Disciplinary Team.

The role will also require the formation of close working relationships with internal and external partners. This will include community Mental Health Teams, Psychiatric In-Patient Services, Children’s Social Care and Paediatric Teams.

The consultant is required to attend an annual job plan review meeting with their Clinical Director and Head of Service. This meeting will normally take place before the end of March each year. The consultant will bring relevant data on workload in the previous year to the meeting, using electronic job planning software (SARD).

The post-holder is expected to offer mental health assessment, psychiatric opinions, diagnosis and holistic care plans for patients up to the age of 18, referred to them in hours and when on call.

The post holder will provide committed, proactive and responsible leadership working with the Intensive Home Treatment and Multi-Disciplinary Team.

The Consultant will provide consultation and advice during allocated programmed activities to the Intensive Home Treatment Team/Crisis team clinicians. The Consultant will review a small number of complex patients for the Intensive Home Treatment Team working jointly with team members.

The Consultant will be responsible for Mental Health Act Assessments in the community and very occasional Section 136 assessments for patients known to IHT or not yet open to the generic teams.

The post holder will be expected to provide appropriate supervision to Junior Medical Staff.

The post holder will be expected to liaise with community teams and Consultants.

Consultant annual leave is arranged with the consultant for EDIHT as appropriate, and thereafter with colleagues in the community teams.

The Children and Young People’s Service currently uses the Care Programme Approach (CPA) and has a relevant all-age Trust policy.

Clinical work would be expected to be carried out wherever felt most appropriate –, at the young person’s home, at a clinic base or another location - for example a young person may have strong feelings about being seen in an alternative community setting. There will also be infrequent occasions when young people need to be seen in other settings such as the Trust’s crisis assessment suite, acute hospital, police station or a residential placement.

The post-holder should write reports for and attend relevant multiagency meetings for young people open to the intensive home treatment team; this may include submitting Form 1 applications for inpatient admission, or providing diagnostic reports. They should also be prepared to work closely with multiagency partners including GPs, education, social care and paediatrics.

Section 18 describes a possible working week.

The post-holder will be expected to follow the Trust’s person centred pathways of care. The Trust is working hard to integrate full use of clinical routine outcome measures (ROMs) and the post-holder would be expected to use these when applicable. Clinical notes are kept within Paris, the Trust’s electronic notes system.

The post-holder will work alongside the multi-professional team and with partner agencies to offer mental health services for their patients. The post holder will continue with lifelong professional learning and development as agreed with their professional development group, appraiser, clinical director and head of service.

The appointee will be expected to undertake the ordinary administration duties of a Consultant post including maintaining good clinical records using the Trust’s electronic record system. They are expected to undertake administrative duties associated with the care of patients, and to record clinical activity accurately and comprehensively.

It is expected that the post holder will participate in professional audit, business planning and related activities. Consultants take a major role in the planning and management of services. There are opportunities for professional development in medical management.
