Job summary

The Darlington CMHT is split in two teams West and East and the two team cover Darlington localities. The caseload for East is 393 and for West is 575 total of 968. The Clozapine for East is 28 and West is 31 total of 59. The Lithium for East is 20 and for West is 49 total of 69. The Depot injections for East is 59 and West is 23 total of 82. The CTO for the East is 8 and for the West is 4 total of 12.

Initially there will be no expectation of the successful candidate doing Medico-legal work but eventually working within capabilities and supervised by Dr Karrar, some Medico-legal work could be exchanged for patient review work. If the successful candidate wants/needs to complete Royal College of Psychiatry examinations or collect evidence for CESR, this would be supported.

The two teams are currently based at the same building in West Park Hospital Darlington and are ensuring close and regular liaison between the two teams. Both teams have their own Consultant Psychiatrist, Team Manager and Advanced Practitioner and Graduate Doctors. All the community adult mental health teams in the Durham & Darlington locality are Generic Mental Health teams.

Job description

Job responsibilities

The Specialty Doctor will work as part of a multi-disciplinary team and provide clinical expertise for both new and ongoing cases particularly issues around psychopharmacology, diagnosis, risk assessments including liaison with the local GPs and inpatient units. All referrals for inpatients will be seen initially by the Crisis Team who are based nearby. West Park Hospital is the the receiving mental health inpatient unit.

Most outpatient work will be at West Park Hospital however there will be opportunity for home visits with the care co-ordinator and also liaison with West Park Hospital for instance formulation meetings and pre-discharge meetings.

The patients will generally be seen with their care co-ordinator and although the Specialty Doctor will be able to work autonomously and independently in terms of decision making, there will be close and ready supervision available from the Consultant Psychiatrist. There is also a Core Trainee attached to this Team who is supervised by Dr Karrar.

As a Specialty Doctor you will be required to attend an annual job plan review meeting with your Associate Medical Director or Lead Pychiatrist and managerial equivalent. This meeting will normally take place before the end of March each year. You will bring relevant data on workload in the previous year to the meeting, using electronic job planning software (SARD).

There is a need for a flexible approach to the pattern of working which may change from time to time in line with the demands from various areas of the service and needs of the population. Changes will not be made to your responsibilities without full consultation.

The Trust has a well-established appraisal system which is monitored by the Responsible Officer and the Trust Board, with the Trust being the designated body. Support and training will be available through the Trusts Revalidation Team to help you become acquainted with the appraisal process and system.

In order to meet the requirements for revalidation, the Specialty Doctor is expected to engage in appraisal by attending an annual appraisal meeting and by maintaining an electronic appraisal portfolio (SARD). This portfolio of supporting information will cover the 4 broad headings outlined by the GMC:

The date of your first annual appraisal will be determined by the Lead Psychiatrist in their meeting with you - two months after you start. Appraisers are allocated by the Revalidation Team.

Induction in using SARD for job planning and appraisal is provided.

You will be encouraged to seek a mentor to support your development in your first year of appointment. The Trust has a trained group of mentors for you to choose from.

Prior to commencing post, you will be informed of the corporate and local induction processes and programme. It is expected that you will participate fully in this and keep up to date with mandatory and statutory training on an annual basis.

Upon commencement, you will be expected to participate in the middle tier on-call rota for South Durham covering specialties. You will receive an on-call availability supplement calculated in accordance with Schedule 12 of the TCS. The level of supplement will depend on the frequency of your participation on the rota.
