Job summary

This is a replacement post arising after the current post-holder moved for reasons of career progression.

The York Psychiatric Liaison Team (YPLT) and the Mental Health Assessment and Liaison Team (MHALT) for older people are managed as part of the TEWV MHSOP directorate (although medical management now sits under Liaison and Specialist Services

The post is based entirely in York Hospital, working as a member of both MHALT and YPLT.

YPLT provide 24 hours a day, 7 days a week, self-harm and mental health assessment and treatment to all adults aged 18 and over within York Hospital Emergency Department and for inpatients age 18-65. They provide out of hours cover for inpatients over age 65.

YPLT also provide assessment and advice for all substance misuse referrals for patients age 18-65.

Referrals for patients under 18 are made to the CAMHS Crisis Team. In exceptional circumstances YPLT may provide assessment for patients age 16-17 within the York Hospital Emergency Department.

YPLT will also provide supervision, teaching and support for multidisciplinary professionals working within YH, with individual link roles developed as required.

YPLT attend monthly multidisciplinary, multiagency high intensity user meetings, where complex cases are discussed and individual care plans developed to address each patients needs, with the aim of reducing high frequency use of services.

Main duties of the job

The post-holder will participate in the mental health assessments of patients in York Emergency Department and inpatients in York Hospital including taking full psychiatric histories, review of clinical records, mental state examinations, risk assessments, making diagnoses and formulating management plans.

The post-holder will also be part of the leadership team within both YPLT and MHALT.

Clinical supervision will be provided by the YPLT and MHALT consultants, or their covering consultants. The post-holder should have a good understanding of the use of the Mental Health Act and Mental Capacity Act, and be approved under Section 12 or able to achieve within 6 months of appointment. The post-holder will be expected to be able to do Mental Health Act assessments and make medical recommendations.

There will be opportunistic teaching of team members and students and the opportunity to participate in teaching programmes both within TEWV and to acute trust colleagues.

The role will be split across both teams and some flexibility may be required at times to fulfil the differing requirements of each team particularly when consultants are on leave.

The post-holder is expected to maintain or establish good links with colleagues within and outwith York Hospital to promote collaborative working. The post-holder will work closely with YPLT and MHALT to develop and improve York Hospital liaison service provision across all ages.
