Job summary

Heatlh Care Assistants interested in Physical health and self-guided interventions are invited to apply for this great opportunity to join the new HUB Team within Darlington Community Mental Health Service.

This is a great opportunity to develop a new service to improve patient care and involve other services within the community to work collobratively together.

The successful candidate must have completed the Diploma in Health and Social Care Level 3 or equivalent. Candidates must also have Literacy and Numeracy level 2 or equivalent and must complete the Trust Healthcare Assistant competencies within their probationary period (first 6 months in role).

Main duties of the job

The main role in the new HUB will be to work delivering physical healthcare interventions to the service users in clinics or home visits where required. The post holder will obtain physical health measurements for monitoring to optimise health and well being for service users. This includes: venepuncture, ECG's, blood pressure, temperature, pulse and weight/BMI.

The post holder will also support the service user in other ways in the recovery focused pathway delivering self-guided interventions.

    • Referral from qualified practitioners to ensure haematological tests are carried out in line with national guidelines
    • Completing pathology request forms
    • Booking appointments in accordance with existing workload of self and other members of the

    • Recording interventions in patient electronic notes and databases
    • Processing pathology results in accordance with local.
  • Maintains, safety, privacy and dignity of all patients in the delivery of patient centred care recognising and respecting differences including spiritual and cultural.
  • Contributes to the creation of a physical and psychological environment conducive to the provision of high quality.
  • Performs ECG monitoring as required
  • Accurately records and reports physiological observations (e.g. temperature, blood pressure, pulse).
  • Identifies changes in patients presentation or monitoring and acts, reports and documents appropriately.
  • Communicates monitoring results with the treating team, including any abnormal results.
