Job summary
This post is only open to those candidates who already have their NMC registration in place and have successfully completed the preceptorship programme. Please note: Newly qualified applicants or those due to qualify will be recruited via a separate process.
An exciting and rewarding opportunity has arisen, for a Band 5 Learning Disability Nurse/Care Co-ordinator working within an established Integrated Team working alongside an MDT which includes Social Workers Nursing, Psychiatry, Psychology, Physiotherapy, Occupational Therapy, Speech and Language Therapy and administrative staff.
Durham County Council are the lead authority of the Integrated Learning Disability Teams. Candidate's will use the Durham County Council system Azeus as their primary recording system and will still have access to TEWV recording systems, training and NHS mail.
The team work collectively and collaboratively with others to ensure a high standard of care is provided and are always willing to go above and beyond what is expected when required, and to ensure people with a diagnosed learning disability and their families and carer are supported by delivering high quality clinical care utilising the Care Act and TEWV clinical Pathways.
The service is currently 9am to 5pm Monday to Friday.
The post holder will also be part of the Nurse on Call rota that covers out of hours 8pm to 8am.
Main duties of the job
The successful candidate will need to have a genuine desire to improve the mental health and well-being of adults with learning disability and behaviours that challenge. They will work as part of a MDT delivering high quality secondary care and enhancing the community services, working with individuals who require specialist interventions using trauma informed care and a recovery-based approach.
Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.

The successful candidate will:
- Hold a caseload, completing assessments, delivering treatment and interventions, monitoring and evaluating outcomes, knowledge and understand of the Care Act and implementing Clinical Care Pathways.
- Maintain good communication, documentation, providing reports and information within agreed timescales.
- To adhere to legal frameworks/ policies and procedures e.g. Care Act Safeguarding, Mental Capacity Act and Mental Health Act
- Maintain their NMC Registration/ PIN ensuring the compliance with the NMC Code/ revalidation.
