Job summary

We are looking for a highly motivated person to join MHSOP Redcar & Cleveland community team to join our busy team.

As a member of the team, primarily working within the organic team, you will be expected to have knowledge of the Dementia Care Pathway and how the process of assessment, diagnosis, treatment and follow up in relation to this.

We encourage discussions within the huddles regarding patients, carers and treatment. Within this role we will support you to expand on your knowledge and expertise with opportunity to progress.

MHSOP Redcar and Cleveland team will support with short term interventions in collaboration with partner agencies and voluntary partners.

You will be a valued member of the teamand will develop excellent working relationships with our partners and stakeholders. This innovative approach will provide opportunities for growth in your practice and professional development.

You can expect regular supervision and support from a very cohesive and supportive leadership team. The service currently operates Monday-Friday 9-5pm.

Main duties of the job

As a member of the team you will provide a service to those individuals who present with mental health issues, with an emphasis on psychologically informed brief interventions and the Dementia Care pathway.

You must be willing to work collaboratively with service users of varying ages and differing needs as well as their carers and families .

You will need to have significant experience of working with people with mental ill health in a care setting, delivering clinical interventions specific to care pathways.

You will possess excellent communication and interpersonal skills. You are required to have the ability and experience to manage complex cases in partnership with other agencies providing care coordination in line with CPA. In addition you will further develop skills in psychological therapies, psychosocial interventions and care management.

You will encourage ongoing engagement from all staff utilising effective leadership skills. You are required to use your own initiative and make decisions independently for this post.

There is an expectation of commitment to the development of high quality care, and to engage in quality improvement activities. You will be supported with regular management and clinical supervision.

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes.
