Job summary

Mental health older persons Darlington Community Team are looking for a Mental Health Practitioner to join our amazing team.

We provide complex interventions, supporting people with severe and enduring mental health conditions .

You will be enthusiastic and highly motivated with experience in the delivery of high-quality planned care. You will be a valued member of the team and will develop excellent working relationships with our partners and stakeholders.

You can expect regular supervision and support from the collective leadership.

Main duties of the job

As a member of the team you will provide a service to those individuals who present with mental health issues.

You must be willing to work collaboratively with service users of varying ages and differing needs as well as their carers and families .

You will need to have significant experience of working with people with mental ill health in a care setting, delivering clinical interventions specific to care pathways.

You will possess excellent communication and interpersonal skills. You are required to have the ability and experience to manage complex cases in partnership with other agencies. In addition you will further develop skills in psychological therapies, psychosocial interventions and care management.

You will encourage ongoing engagement from all staff utilising effective leadership skills. You are required to use your own initiative and make decisions independently for this post.

There is an expectation of commitment to the development of high quality care, and to engage in quality improvement activities. You will be supported with regular management and clinical supervision.
