Job summary
We are seeking two highly motivated enthusiastic and passionate Learning Disability Nurse's to join the DTV ALD Physical Health Team as a Band 5 Community Nurse / Health Facilitation Nurse. One will be bases within Teesside and one to be based in Durham.
The team provides specialist advice and support to both people with a learning disability and to health services to reduce the health inequalities and premature mortality that people with a learning disability face.
This role is for full time permanent position within the team and the working hours are Monday - Friday 9am-5pm. As part of the role, you are also required to participate in the Adult Learning Disability Nurse on Call (Monday-Sunday 5pm-9am)
This post is only open to those candidates who already have their NMC registration in place and have successfully completed the preceptorship programme.Please note: Newly qualified applicants or those due to qualify will be recruited via a separate process.
Main duties of the job
The successful candidate will work with health colleagues within the wider Learning Disability Community Teams, Local Authority colleagues and in liaison / partnership with primary and secondary care services, using a Person-Centred Approach to enable and support adults with a learning disability and their carers to promote the health needs of people with a learning disability and enable access to wider health services.
