Job summary
The Forensic Community Service is looking to recruit a highly motivated Band 4 Community Associate Practitioner to join their multi-disciplinary team. The service provides assessment and treatment for people who are within the Secure Inpatient Service and the community.
This post is also open to those who have an NVQ Level 3 in Health and Social Care and are agreeable to completing the foundation degree within an agreed timescale.
We are looking to recruit an Associate Practitioner to form an integral part of our service, working under the supervision of the multi-disciplinary team. This role also provides the opportunity for close working with support workers from our partner organisation, The Richmond Fellowship.
The service is looking to appoint an individual with a range of transferrable skills, who is highly motivated and enthusiastic about working with this patient group. In exchange the successful candidate will be well supported within a friendly multi-disciplinary team, with continuing opportunities for their ongoing professional development.
The Forensic Community Service is based at Roseberry Park hospital; however, we do support people within a wide geographical area.
Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.
Main duties of the job
To work as integral part of the Forensic Community service supporting patients in their transition from Secure Inpatient Services to the community and post discharge.
To improve the lives of people with mental ill health by minimising the impact of their condition through the delivery of excellent services to promote recovery and well being.
To be compassionate in meeting the needs of patients and their carers.
To deliver patient care, as directed by registered practitioners and the team manager, undertaking a range of clinical/therapeutic interventions with limited or indirect supervision in patients' homes and other community settings.
To promote social inclusion, community access and participation through the provision of patient care.
