Job summary

An exciting opportunity has arisen for a Nurse to join the East Integrated Community Mental Health Team based in Northallerton.

This post is only open to those candidates who already have their NMC registration in place and have successfully completed the preceptorship programme. Please note: Newly qualified applicants or those due to qualify will be recruited via our recruitment fairs.

We are a forward thinking team teamwork that are looking for an individual who is creative, motivated and passionate about providing excellent patient care. We are a multi disciplinary team including a Consultant, Psychologists, Occupational Therapist, Mental Health Nurses and Learning Disability Nurses and support workers. Within the team we enjoy promoting wellbeing and a positive work life balance and have supported several newly qualified nurses through their preceptorship.

As a core member of the team, you will also be responsible for:-

  • providing assessments
  • Managing a caseload
  • Monitoring Physical Health
  • delivering intervention in the community.

You will ensure that the service users' needs are:-

  • identified and central to their collaborative care plan, well-being and recovery plans
  • promoting social inclusion
  • independence and well-being

You will demonstrate:-

  • Excellent assessment skills
  • interpersonal and team working skills
  • be able to work collaboratively with a range of statutory and third sector agencies.

Main duties of the job

To improve the lives of people with mental ill health by minimising the impact of their condition through the delivery of excellent services to promote recovery and well being.

To be compassionate in meeting the needs of patients and their carers.

To work as part of a community team which may be integrated, workingcollaboratively and in partnership with local authority services.

To be professionally accountable and responsible for patient care undertaking a range of clinical/therapeutic interventions in patients' homes and other community settings.

To promote social inclusion, community access and participation through the provision of patient care.

To facilitate others in the team to develop competence by providing leadership, day to day supervision, clinical advice and clinical supervision to associate practitioners, community support workers and students as appropriate.

To be committed to and involved in activities integral to the Trust's QualityImprovement System.

To promote at all times a positive image of people with mental ill health.

To promote at all times a positive image of the Adult Mental Health Service and the wider Trust.
