Job summary
Consett Community Mental Health Team are actively looking for a Band 5 Community Mental Health Nurse to join our team. We are passionate about teamwork and in creating a great place to work. We are happy to support newly qualified nurses with their preceptorship and you will have a thorough induction programme offered.
As a core member of the team, you will also be responsible for:-
- your own caseload
- providing assessments
- care co-ordination
- delivering intervention in the community.
You will ensure that the service users' needs are:-
- identified and central to their CPA Well-being and Recovery plans
- promoting social inclusion
- independence and well-being
You will demonstrate:-
- Excellent assessment skills
- interpersonal and team working skills
- be able to work collaboratively with a range of statutory and third sector agencies.
Main duties of the job
To improve the lives of people with mental ill health by minimising the impact of their condition through the delivery of excellent services to promote recovery and well being.
To be compassionate in meeting the needs of patients and their carers.
To achieve successful completion of the Preceptorship Program.
To work as part of a community team which may be integrated, workingcollaboratively and in partnership with local authority services.
To be professionally accountable and responsible for patient care undertaking a range of clinical/therapeutic interventions in patients' homes and other community settings.
To act as lead professional or care coordinator as appropriate.
To promote social inclusion, community access and participation through the provision of patient care.
To facilitate others in the team to develop competence by providing leadership, day to day supervision, clinical advice and clinical supervision to associate practitioners, community support workers and students as appropriate.
To be committed to and involved in activities integral to the Trust's Quality Improvement System.
To promote at all times a positive image of people with mental ill health.
To promote at all times a positive image of the Adult Mental Health Service and the wider Trust.
