Job summary

An opportunity has arisen in the York Outreach Recovery Team for a Senior Mental Health Practitioner to work within the 'out of area' team which is devoted to the repatriation of service users from in patient rehab hospitals back into their local area in the community.

Applications are invited from those with a relevant nursing qualification or allied professional qualification, who has the ability to work independently and within a team. The post holder will be responsible for supervised care co-ordination and co-working of identified clients in conjunction with private sector partners. It is vital that the applicant has previous experience working mental health

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.

Main duties of the job

The successful applicant with hold a small caseload to enable focused and specific interventions concentrating on returning to community care. There will be a great emphasis of multi-agency collaboration and inter-agency partnership working. With the majority of the inter facing being with private sector care providers, the applicant must have experience working with other agencies such as within the third sector, private sector, statutory or primary care sector.

As part of the out of area team there is a designated social worker and another senior mental health practitioner alongside leadership oversight and support, regular MDT reviews of the caseload and both regular clinical and managerial supervision. Inter team collaboration is integral to this role to enable the appropriate and timely discharge of service users to their local area to continue community care. Knowledge of funding and 117 aftercare planning would be beneficial alongside knowledge of care providers and their requirements would also be beneficial.

  • Recognised sign-off mentor, clinical educator or equivalent. Must be achieved within agreed timescale.
