Job summary

Stanley Community Mental Health team is a newly created team following a restructure of community teams in Durham. We are looking to expand our current workforce.It is an exciting time to join the team as we work together to embed a newly created pathway with a strong focus on patient centred trauma informed care.

We are actively looking for Band 6, Senior Community Mental Health Practitioner to join our MDT.

Main duties of the job

The post is for a full time duty worker, who will cover duty and oversee a caseload of lead professional cases on annual reviews for shared care medication

Senior Practitioners will need a good understanding of collaborative practice and in working with adults of varying ages and complex needs, including service users, their carers' and families.

As a core member of the team, you will also be responsible for:-

You will ensure that the service users' needs are:-

  • identified and central to their CPA Well-being and Recovery plans
  • promoting social inclusion
  • independence and well-being

You will demonstrate:-

  • excellent assessment skills
  • interpersonal and team working skills
  • be able to work collaboratively with a range of statutory and third sector agencies.

The team embraces changes to improve the quality and effectiveness of the service provided and as a senior practitioner you would be expected to demonstrate leadership skills to bring these about. As a Band 6 you will be expected to take on day to day support of the team and offer supervision to other staff and students.

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes.   • Current professional registration with an approved professional body.
  • Evidence of Continuing Professional Development.
  • For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education. Must be achieved within agreed timescale.
  • Recognised sign-off mentor, clinical educator or equivalent. Must be achieved within agreed timescale.
  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent)

Desirable

  • Leadership or management qualification
  • Post-graduate qualification in a relevant area

Experience

Essential

  • Significant experience working with people with mental ill health in a care environment.
  • Working collaboratively with service users and their families/carers.
