Community Mental Health Practitioner - Interventions & Hub Working

This post is being advertised as x1 permanent and x1 - 12 months fixed term to address a temporary need, however the Trust has made the commitment to offer the successful applicant a permanent contract with the Trust. In the event that the need for this post ends, a suitable alternative post will be offered at the same band. Come & join The South Tees community team based at Foxrush in Redcar who are beginning an exciting community transformation of services. We are looking for Mental Health Practitioners to join our journey to change and improve the local community's initial contact with our services with a "no wrong door approach".

We encourage and welcome discussions and the opportunity to visit our new working multi agency bases. Within this role we will support you to expand on your knowledge and expertise with opportunity to develop special interests such as substance misuse and psychological interventions.

We are currently in a period of transition from traditional Psychosis and Affective Disorders teams moving towards a blended approach of Hub and Intervention teams.

The interventions team will provide complex interventions supporting people with severe and enduring mental health conditions whilst our identified multi agency community hubs will support with short term interventions and sign posting to other partner agencies and voluntary partners.

Main duties of the job

As a member of the team you will provide a service to those individuals who present with mental health issues, with an emphasis on psychologically informed brief interventions.

You must be willing to work collaboratively with service users of varying ages and differing needs as well as their carers and families .

You will need to have significant experience of working with people with mental ill health in a care setting, delivering clinical interventions specific to care pathways.

You will possess excellent communication and interpersonal skills. You are required to have the ability and experience to manage complex cases in partnership with other agencies providing care coordination in line with CPA. In addition you will further develop skills in psychological therapies, psychosocial interventions and care management.

You will encourage ongoing engagement from all staff utilising effective leadership skills. You are required to use your own initiative and make decisions independently for this post.

There is an expectation of commitment to the development of high quality care, and to engage in quality improvement activities. You will be supported with regular management and clinical supervision.

You can expect regular supervision and support from the collective leadership. The service currently operates Monday-Friday 9-5pm, flexibility and part-time or job share can be considered.
