An exciting opportunity has risen for a Band 6 Senior Mental Health Practitioner to join the Community Rehabilitation and Recovery Team across Durham and Darlington. Due to recent investment you will be joining us at an exciting time to develop the service to meet the requirements of the Community Mental Health Framework (CMHF).
We are looking for an enthusiastic and dynamic individual who are able to work with the wider MDT, voluntary sector and third sector organisations
Main duties of the job
Key skills and responsibilities will include working in rotation with other senior Practitioners within the team to carry out Rehabilitation assessments to determine the needs of the individual and how these can be met by the team. You will be responsible for having oversight of Rehabilitation bed availability and supporting patient flow from inpatient settings as well as working alongside community mental health teams in assessing service user needs for community rehabilitation interventions. This will guarantee continuity of care for service users transiting into and out of rehabilitation services. You will be responsible for providing coordinating and delivery an individualised support package that will enhance their skills and keep them sage and well in the community. Effectively liaising with referring agents will be crucial within your role. The role will involve producing robust treatment reports and recommendations in accordance with Trust and local policies.
It is essential that you exemplify excellent interpersonal skills as they will be paramount when establishing close working relationships with all appropriate agencies and networks in the field of Mental Health, locally, regionally and nationally. There will be an expectation that you will have a small clinical caseload working alongside the Community Rehab and Recovery team in delivering specialist rehabilitation interventions.
