Job summary

An exciting opportunity has arisen for a Senior Community Mental Health Practitioner to join North Dales Community Mental Health Team- fixed term contact to cover 12 months maternity leave.

This post is being advertised to address a temporary need for 12 months however the Trust has made the commitment to offer Registered Nurses a permanent contract with the Trust. In the event that the need for this post ends, a suitable alternative post will be offered (this does not apply to other registered professions). Applicants should be aware that alternatives may require some flexibility on role and/or location.

Main duties of the job

Senior Practitioners will need a good understanding of collaborative practice and in working with adults of varying ages and complex needs, including service users, their carers' and families.

As a core member of the team, you will also be responsible for:-

  • your own caseload
  • providing assessments
  • care co-ordination
  • delivering intervention in the community.

You will ensure that the service users' needs are:-

  • identified and central to their CPA Well-being and Recovery plans
  • promoting social inclusion
  • independence and well-being

You will demonstrate:-

  • excellent assessment skills
  • interpersonal and team working skills
  • be able to work collaboratively with a range of statutory and third sector agencies.

The team embraces changes to improve the quality and effectiveness of the service provided and as a senior practitioner you would be expected to demonstrate leadership skills to bring these about. As a Band 6 you will be expected to take on day to day support of the team and offer supervision to other staff and students.

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.

  • Current professional registration with an approved professional body.
  • Evidence of Continuing Professional Development.
  • For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education. Must be achieved within agreed timescale.
  • Recognised sign-off mentor, clinical educator or equivalent. Must be achieved within agreed timescale.
  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent)

Desirable

  • Leadership or management qualification
