Job summary
This post is being advertised to address a temporary until 31st March 2023 need however the Trust has made the commitment to offer Registered Nurses a permanent contract with the Trust. Applicants are invited to apply for a new and exciting opportunity to work within Durham County Council's 14-25 Navigation Team. The team work with children and young adults with complex needs including learning disabilities, physical disabilities, autism and mental health difficulties to achieve a high quality of life and optimum health.
This role will provide an excellent opportunity for candidates to contribute to the integration of our team, working alongside Social Workers to ensure appropriate health provision is in place as the child / young person transitions from Children to Adult services.
This unique role provides the opportunity to gain knowledge, skills and experience across health and social care, in both children and adult services. The experience gained will undoubtedly contribute to professional development and evidence excellent leadership skills for those wanting to further progress their career.
The role is based at Durham Leadership Centre, Spennymoor however the team adopt a hybrid model of working.
Main duties of the job
To improve the lives of children and young people by ensuring holistic, person centred care and support.
To lead upon strengthening the transition of children and young people requiring a commissioned service into adulthood.
To consider the child / young person's health needs and ensure they are on the correct pathway in relation to health provision.
To be compassionate in meeting the needs of clients and their carers.
To promote integrated and mutli-agency working to ensure the needs of the child / young person are met.
To be professionally accountable and responsible for client care, undertaking a range of interventions in a variety of community settings.
To support discharge planning for children and young people detained under the Mental Health Act (1983).
To contribute to the development and learning of social care colleagues through providing clinical advice and guidance.
To assess, construct, monitor and review the care and support an individual requires.
To promote social inclusion and independence.
To demonstrate an awareness of key legislative frameworks that cover both children and adult services including the Care Act (2014), Mental Capacity Act (2005), Children Act (1989, 2004) and the Mental Health Act (1983).
To be responsible for recognising the potential signs of harm, abuse or neglect and report accordingly.
