The COVID resilience rehabilitation team is a collaborative between Tees Esk & Wear Valleys NHS Foundation Trust and Pioneering Care Partnership and 8 voluntary sector organisations. We are looking for a fitness practitioner to join our team. As a Healthy Living Advisor the post holder will work with citizens of County Durham who have been affected either directly or indirectly by COVID to help them overcome the impact of the pandemic by promoting and supporting them to achieve healthy lifestyles. You will be based at Inglewood at Lanchester Road Hospital in Durham and work into Pioneering Care Partnership at Newton Aycliffe.
Applicants must ensure that prior to applying they have agreement from their current line manager that they can be released on a secondment basis.
In the event that permanent funding is secured for this post the successful applicant will be confirmed in post on a permanent basis.
Main duties of the job
The main aim of the post is to work with citizens of County Durham who have been affected by COVID to promote physical activity to overcome isolation, improve their general wellbeing and help them manage their symptoms where applicable. You will work with people on a 1:1 basis or in groups as deemed appropriate. You will deliver healthy living interventions to the citizens to help them improve their mental and physical wellbeing by providing,

- health promotion and physical interventions
- signposting people to activities provided by others in Co Durham which support their health and wellbeing
About us
The team consists of Resilience Leads; Psychologists and Psychology Assistants; 12 Resilience workers and representatives from 8 voluntary sector organisations. You will have monthly supervision from a qualified physiotherapist and where necessary a dietitian where you have protected time to celebrate your successes and reflect on any issues. You will be one of a supportive network of 10 fitness practitioners already working in TEWV.

Under the guidance of a qualified physiotherapist and/or dietitian you will
- Complete initial health assessments for service users referred to the service
- Provide advice and guidance on healthy eating, first line weight management, smoking cessation, brief interventions for alcohol, activity advice and exercise prescription.
- Work collaboratively with individuals to develop an action plan to support / promote behaviour change and motivate the individuals to sustain those behaviours, leading to more healthy choices and actions.
- Support individuals action plans over an agreed period of time.
- Review and revise action plans with individual as appropriate
- Take service users into the community to access local services
- Manage conflict / upset (should this occur), as needed
- Deliver safe, effective and evidence based healthy living sessions specific to their area of expertise, within different locations including carrying out risk assessments of facilities to be used.
- Hold your own caseload independently providing personalised healthy living programmes for individuals based on their assessment and modify them according to their need according to their area of specialist knowledge i.e. smoking cessation, nutrition, drug or alcohol use. This includes the design, delivery, monitoring and modification of exercise programs for individuals and for groups.
- Within the limits of own scope of practice to assess users of the service and decide if fit to exercise, and decide on type of exercise and course of action
- Contribute to the implementation of the physical health improvements as part of
- Contribute to the development work of the service working in partnership with other members of the multi-disciplinary team, acute NHS services, local community organisations, voluntary sector and groups as appropriate to needs of the service and service users
- Must be a persuasive advocate for physical healthcare and healthy lifestyles, motivating and encouraging others to participate in therapeutic or recreational activities to promote healthier lifestyles
- Be expected to make decisions related to prioritising your own workload
- Recognise own limitations and makes judgements within level of own competency when undertaking patient assessment, physiological measurements and therapeutic interventions, in order to modify intervention plans within agreed parameters.
- Recognise unpredictable or non-routine situations, taking appropriate action within agreed parameters and decides when to escalate issues to more senior staff.
