Community Mental Health team in Redcar as looking for a highly motivated Band 5 Community Nurse. This is an exciting time to join the community mental health teams with the transformation of our community services currently under way.
This is an exciting opportunity for new or experienced practitioners to work as part of a multi-disciplinary team in the Redcar & Cleveland locality. As a Community Psychiatric Nurse you will have a genuine interest in collaborative working with people of varying ages and varying needs, including service users, their carers and families.
You will have an enthusiastic approach towards your own personal and professional development. In return, you will receive regular appraisal, clinical, management and caseload supervision and will benefit from the support of an experienced and committed multi-disciplinary leadership team, including the Team manager and the Advanced Practitioner.
Main duties of the job
As a core member of the team, you will also be responsible for your own caseload, and delivering interventions in the community. In line with transformation we are moving towards teams who offer the right intervention and the right time and therefore the work will be a blend of treatment pathways.
You will ensure that the service users' recovery goals are identified and central to their well-being and recovery plans, promoting social inclusion, independence and well-being. You will demonstrate excellent assessment skills, interpersonal, and team working skills and be able to work collaboratively with a range of statutory and third sector agencies.
The team is embracing a number of changes to improve the quality and effectiveness of the services provided in the community to our patients and as a community psychiatric nurse you would be an essential element in facilitating this change.

Redcar & Cleveland covers both rural and coastal a town geographical locations on the North east coast. The teams are currently currently at Kirkletham business park but we are excited to be looking for bases within our communities to deliver interventions within our local Hubs.

Clinical Responsibilities, Patient ContactMaintains safety, privacy and dignity of all patients in the delivery of patient centred care, recognising and respecting differences including spiritual and cultural beliefs.Takes steps to obtain patient consent to care and treatment in accordance with the Mental Capacity Act, ensuring that patients who lack mental capacity remain at the centre of decision-making and are fully safeguarded.Acts, wherever applicable, in accordance with the Mental Health Act and associated policies and procedures.
Responsible for recognising the potential for or signs of patient harm, abuse or neglect, including poor clinical practice, reporting all such concerns and taking all reasonable steps to protect the patient. Responsible for identifying and reporting concerns regarding the safeguarding of children who may be at risk.Ensures the highest professional standards and attitudes towards the care of patients from all staff.
