The Darlington Adult Community Mental Health services are embarking on a transformation which will see the community team work along side our primary care colleagues, GP, voluntary agencies and an of the other services in Darlington to provide the right care, with the right practitioner in a timely manner. We are changing the way we work so that our service users, families, carers get the best service that we can offer, and ensure that we have skilled trained practitioners to provide the right interventions.

Main duties of the job

As part of the new ways of working your role could be in the HUB or Intervention team which we are in the process of developing now. In the HUB team - this will be working as part of the primary care services to triage, assess and signpost to the most appropriate service within the community. The HUB will have a dedicated Psychical Health Team, for patients on specific medications, as well as completing the general required observations. In addition this team will deliver low level one to one work along with ungoverned group interventions.

The Intervention Team will be staff trained to work with the complex patients that require more specific interventions to meet their mental health needs. As we move away from Care Coordination, and with this new model we envisaged a team that will be therapy focused and highly trained.

Within both of the new teams you will be required to provide support to individual service users, their carers and wider family, if required. This role will involve direct work with Adults with variety of mental health difficulties, some of them are: depression, anxiety, eating disorders, self-harm behavior, suicidal ideations/attempts to end life, personality disorders to just name few.
