We have an exciting new opportunity for an experienced pharmacist to lead the development and delivery of place based medicines hubs in Durham Tees Valley. In the first instance you establish two pilot medicines hubs: directly delivering the services in Hartlepool Primary Care Network (PCN) and managing the pharmacist in Medicines Hub Chester-le-Street PCN.

Main duties of the job
- To lead and manage the development and delivery of Medicines Hubs place based across Durham and Tees Valley.
- To line manage Pharmacists and Pharmacy Technicians and consider the skill mix of roles within the Medicines Hub.
- To lead on designated areas of pharmacy responsibility across the whole trust
- The post holder will work with all professional staff, patients and carers in the provision of community place based clinical pharmacy service, ensuring high quality patient focused pharmaceutical care
- To deputise for the Deputy Chief Pharmacist and Chief Pharmacist and be involved in pharmacy service developments as needs or opportunities arise
- To be engaged in delivering the Trust's medicines optimisation strategy / framework, in particular the integration of pharmacy professionals into community teams and primary care
- The post holder will be engaged in an aspect of the place based clinical pharmacy service
- To promote at all times a positive image of people with mental health conditions and learning disabilities.

This post is split 80:20 between Community Transformation in Durham Tees Valley and Pharmacy Leadership respectively.
You will be working with healthcare professionals from the pharmacy and community teams within the Trust's Durham Tees Valley and Forensics Care Group, Primary Care Networks and partners organisations across wider care system.
You will work closely with the wider pharmacy leadership team to influence, develop and lead, when necessary, Trustwide projects and service developments.

Internally:
- Information regarding clinical and cost effective prescribing in line with evidence based practice and disseminate information in a form most likely to influence clinical behaviour
- Highly complex, sensitive or contentious information where co-operation/ agreement is required with senior managers, other staff, partner organisations and user/carers at all levels e.g. funding for new clinical services
- The post holder will lead/chair working groups as appropriate in the field of medicines optimisation, medicines safety, service development, standard delivery of clinical service
- Effectively resolve informal or verbal complaints in the first instance before appropriate referral to the Chief Pharmacist. Support the Chief Pharmacist in the investigations of formal complaints.

- To develop and sustain constructive partnership relationships with a broad range of organisations and individuals including Clinical Commissioning Groups (CCGs), Primary Care Networks (PCNs), Local Authority, Acute Trust pharmacy services, private sector providers of pharmacy services, General Pharmaceutical Council, College of Mental Health pharmacists, police
- Liaises with CCGs, PCNs, GPs and community pharmacists to ensure the pharmacological management of patients is not compromised by organisational boundaries
- To maintain a strong network of pharmacy contacts, as agreed with the Chief Pharmacist, at local, regional and national and Department of Health levels in order to pro-actively influence the specialist mental health pharmacy agenda

- To be actively involved in the pharmacy services for patients on clozapine, lithium and medicines identified in patient safety alerts
- Assist the other Lead Pharmacists in analysing information and report to appropriate committees making recommendations for changes in practice and procedures

- Prepare business cases for/ costing out new clinical pharmacy services and developments
- To support the Chief / Deputy Chief Pharmacist to maintain effective clinical pharmacy service arrangements across the many Trust sites ensuring the Trust responds to new evidence/ safety advice about medication and raising the profile of the clinical pharmacy service related issues (financial, risk and quality) at senior Trust management level
- To participate in activities and programmes, both internally and externally, which enhance the growth, development and improvement of pharmacy and the Trust
