Job summary

An exciting opportunity has arisen for a band 7 Mental Health Practitioner to work into on of the local Primary Care Networks (PCNs) GP practices to support the needs of our local population living with mental health difficulties. This is a new and innovative service supporting people in primary care settings and working in a collaborative and multiagency way to meet the needs of patients with a range of mental health needs.

GPpractices have begun working together alongside community, mental health,social care, pharmacy, hospital and voluntary services in the local area to form PCNs. PCNs signal a significant change in the way practices interact not only with each other, but the wider health andsocial caresystem. This will enable care to be delivered to a defined patient population, in a different way to meet the needs of that population. This will be focused on prevention andpersonalisationof care, making best use of resources collectively.

Main duties of the job

The post will sit within the GP practices and will offer assessment, diagnosis, treatment/brief interventions and review for both working age adults and older people. The role will be offering specialist mental health clinical care. The role will also offer networking expertise and navigation to a range of specialist mental health supports to ensure each person once assessed receives the best possible care in the right place at the right time by the right supporting service.

Candidates should have extensive clinical knowledge and experience across a range of mental health settings. They will need to hold or be willing to work towards a prescribing qualification and be able to offer diagnosis and treatment plans across all mental health conditions and presentations. Candidates will need to be able to offer supervision to other mental health professionals and GP practice staff. Experience in leading meetings, presentation skills and offering training and education to professionals across a range of specialties will be essential.

About us

You will be completely deployed into a PCN to work as part of that PCN MDT, MDT members will include: GPs, practice nurses, social prescribers, health and wellbeing coaches, pharmacists, physiotherapists etc.

You will also be connected to other staff in similar roles across different PCNs with support from and associate nurse consultant through a daily huddle.

To improve the lives of people with mental ill health by minimising the impact of their condition through the timely delivery of an excellent, compassionate services to promote the recovery and well-being of patients and their carers / families.

To work collaboratively with General Practitioners and other practice staff within the locality to address the mental health needs of their adult and older adult practice population.

To be compassionate in meeting the needs of patients and their carers, recognising the importance of co-production and choice in the planning and delivery of care.

To be professionally accountable and responsible for patient care, undertaking assessments and signposting to appropriate services or providing time-limited psycho-social/therapeutic interventions directly, in e.g. GP surgeries, patients homes and other primary care settings. This will include providing expert clinical advice to GPs such as specialist opinion or prescribing support (within the parameters of the NMP framework), and providing supervision and oversight of practice based CPNs.

To carry own specialist caseload of complex cases and act as lead professional as appropriate, providing a pathway management function for other more complex patients within primary care.

To promote social inclusion, community access and participation through the provision of patient care.

To facilitate others in the team to develop competence by providing clinical advice and clinical supervision to practice based mental health practitioner and other practice staff as appropriate or other less experienced primary care mental health workers and students.

To provide specialist clinical advice, consultation and support to other care providers within primary, secondary, voluntary and statutory services as required.

To be committed to the Trusts Quality Improvement System.
