Job summary

Due to Promotion of the current Band 6 to the post of Advanced Practitioner within the Community Mental Health Teams based at West Park Hospital we have 1 x full time vacancy, however will consider part time hours of a minimum of 22.5 hours which can be work flexibly. It is an exciting time in Darlington as we embark on the Transformation of services in line with the 10 year plan. The two current community teams will be moving to a Hub and Intervention Model, this post is advertised as part of the transformation and could sit within either of the teams.

Main duties of the job

Senior Practitioners will need a good understanding of collaborative practice working with adults of varying ages and complex needs, including service users, their carers' and families.

As a core member of the team, you will also be responsible for:-

  • your own caseload
  • providing assessments
  • care co-ordination may only be for a limited time if in the HUB as this will be short term work with patients, in the intervention team this will be longer structured work

You will ensure that the service users' needs are:-

  • identified and central to their Well-being and Recovery plans
  • promoting social inclusion
  • independence and well-being

You will demonstrate:-

  • excellent assessment skills
  • interpersonal and team working skills
  • be able to work collaboratively with a range of statutory and third sector agencies.

The team embraces changes to improve the quality and effectiveness of the service provided and as a senior practitioner you would be expected to demonstrate leadership skills to bring these about. As a Band 6 you will be expected to take on day to day support of the team and offer supervision to other staff and students.

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes.
