Job summary
We are looking for highly dedicated clinicians, to join our new Mental Health Service for Older Persons Intensive Home Treatment and Crisis Team. Our team provides a comprehensive specialist mental health service to patients, families, carers and clinicians in the community covering the area of Durham and Darlington.
This post is being advertised to address a temporary need however the Trust has made the commitment to offer Registered Nurses a permanent contract with the Trust. In the event that the need for this post ends, a suitable alternative post will be offered (this does not apply to other registered professions). Applicants should be aware that alternatives may require some flexibility on role and/or location.
Applicants must ensure that prior to applying they have agreement from their current line manager that they can be released on a secondment basis.
In the event that permanent funding is secured for this post the successful applicant will be confirmed in post on a permanent basis.
Main duties of the job
The role will including supporting patients after discharge from; acute hospitals, acute mental health hospital and intermediate care beds. Along with supporting patients and carers in their own homes and providing telephone support for those who are in crisis.
Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.

As such you will be expected to work a shift pattern which has the potential to include night shift. At present the team work across 7 days. We are looking for fulltime and part time staff, therefore candidates should state their preferred working arrangements in the application form.
This is a challenging but rewarding role, offering an exciting opportunity to those who have a professional registration with the NMC or HCPC. You will need to have strong assessment and therapeutic skills to fulfill this role. You will require the ability to work autonomously whilst still contributing to the team as a whole. Previous experience of working with older people who are suffering from a range of mental health conditions is essential. The role requires working with many other organisations and partners to ensure that the patient receives appropriate care. Therefore the successful candidates must be good at communicating and using a electronic patient record. We offer excellent staff support through clinical supervision and teamwork.
You will have a positive approach to work, with a demonstrable background of providing high quality care and evidence of commitment to professional development. You will have experience and willingness to teach and educate others as well as a commitment to service development. You will be responsible for the leadership of junior staff and health care assistants to produce the best possible outcomes for older people in the community and healthcare settings.
