Job summary

The Forensic Community Team are seeking a Band 6 Community Nurse to join our team. The role will involve supporting people in their transition from Secure Inpatient Services into the community and post discharge. There will also be an opportunity to offer advice and support to Community Mental Health Teams and Adult Inpatient services.

We are looking to appoint someone who is highly motivated and enthusiastic about working with this patient group. In exchange the successful candidate will be well supported within a welcoming multi-disciplinary team, with continuing opportunities for their ongoing professional development.

The successful applicant will be joining the team during an exciting period of change, developing the Forensic Community Service. There will be an opportunity for involvement in these developments.

We welcome applications from registered Mental Health and Learning Disability Nurses.

The Forensic Community Team is based at Roseberry Park hospital; however, we do support people within a wide geographical area. The post holder will be expected to travel.

Main duties of the job

Including but not exclusively:

To improve the lives of people with mental ill health by minimising the impact of their condition through the delivery of excellent services to promote recovery and well being.

To be compassionate in meeting the needs of patients and their carers.

To be responsible for provision of excellence in clinical care, providing clinical leadership to the team and promoting a culture of evidence-based practice.

To be professionally accountable and responsible for formal risk assessment, risk management planning and care of patients, undertaking a range of clinical/therapeutic interventions in patients' homes and other community settings.

To act as care co-ordinator as appropriate.

To promote social inclusion, community access and participation through the provision of patient care.

To facilitate others in the team to develop competence by providing leadership, day to day supervision, clinical advice and clinical supervision to community support workers and students.

To be committed to and take a lead role in activities integral to the Trust's Quality Improvement System.

To promote at all times a positive image of people with mental ill health.

To promote at all times a positive image of the Health & Justice Service and the wider Trust.
