Job summary
Applicants are invited from Registered Mental Health Nurses to apply for a new and exciting opportunity to work within Durham County Council's 14-25 Navigation Team. The 14-25 Navigation Team work with children and young adults with complex needs including learning disabilities, physical disabilities, autism and mental health difficulties to achieve a high quality of life and optimum health. The role is based at Durham Leadership Centre, Spennymoor and the team adopt a hybrid model of working.
The post is being advertised on a fixed term secondment basis to pilot the integration of the 14-25 Navigation Team following investment from the Community Mental Health Framework, the aim of such being to strengthen transition to adult services. Funding has been approved for 12 months at present however there may be the opportunity for this post to be extended.
This post is being advertised to address a temporary need however the Trust has made the commitment to offer Registered Nurses a permanent contract with the Trust. In the event that the need for this post ends, a suitable alternative post will be offered . Applicants should be aware that alternatives may require some flexibility on role and/or location.
Applicants must ensure that prior to applying they have agreement from their current line manager that they can be released on a secondment basis.
In the event that permanent funding is secured for this post the successful applicant will be confirmed in post on a permanent basis.
Main duties of the job
The successful candidate will carry a caseload consisting of individuals between the ages of 14 and 25 years of age who will require a commissioned service and input from integrated adult services post-18.
This is a unique role providing the opportunity to gain knowledge, skills and experience across both health and social care and children and adult services.
Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act (2010).
