A new and exciting opportunity has arisen to support the development of our nursing workforce at HMP Northumberland. This post is a development opportunity for a post preceptorship Nurse to join the team in a development post as a Band 5 where you will be fully supported through a 12 month competency framework into a Band 6 position. HMP Northumberland is a category C prison housing men from who are serving prison sentences from the NE of England.

This vacancy is open to Band 5 Registered Nurses with a minimum of 12 months post qualification experience and successful completion of preceptorship. This is a developmental post with progression to a Band 6 upon meeting the criteria within the Band 6 job description and competency framework specific to Health & Justice Services.

The successful candidate will work across the breadth of the service in order to develop knowledge, skills and experience. Excellent communication and interpersonal skills are required as well and an ability to work under pressure and with initiative. Main duties and responsibilities will include:
- Acts as lead professional for patients and may take responsibility as care coordinator for a delegated caseload of patients on Care Programme approach as appropriate.
- Processesnew referrals and has responsibility for own caseload with support from a Clinical Lead or team manager.
- Assesses patients needs and develops, implements and evaluates intervention plans as part of the multi-disciplinary team, with the involvement of the patient and where appropriate, their carer. Guidance will be provided by a senior clinical lead or advanced practitioner as required, for more complex patients.
- Ensures intervention plans are based on current risk assessment, evidence based practice, critical thinking and whole system support requirements that take account of relevant physical, social, cultural, psychological, spiritual, genetic and environmental factors.
- Undertakes risk assessments in accordance with the Trusts Clinical Risk Assessment and Management Policy, devising and implementing actions and intervention plans which take the risk formulation fully into account.
- Responsible for the safe transport and administration of medicines in accordance with Trust policy, legal requirements and NMC guidance, where professionally applicable. Administers intramuscular injections.
- Takes steps to obtain patient consent to care and treatment in accordance with the Mental Capacity Act, ensuring that patients who lack mental capacity remain at the centre of decision-making and are fully safeguarded.
- Ensures the highest professional standards and attitudes towards the care of patients from all staff. Patients have a range of mental health conditions, may demonstrate behaviours that challenge and may have difficult family situations of relevance.
