Job summary

This is an exciting opportunity to work with the corporate nursing, clinical pharmacy and wider multidisciplinary teams to ensure a high standard of patient care with regard to medicines safety. As a Medicines Optimisation Clinical Specialist Nurse, you will participate and/or lead on developments in clinical practice, with respect to the safe, and effective management of medicines. You will also act as resource and key link between pharmacy, nursing and other clinical staff. It is a great opportunity for you to provide nursing leadership, support and co-ordination for the implementation of the medicines optimisation strategy.

Main duties of the job

The role requires post holder to work in collaboration with the Corporate Nursing Team and Medicines Safety & Governance Pharmacist to promote and improve medication safety throughout the trust. You will be expected to provide a nursing lead to disseminate incident learning and assist in implementation of error reduction strategies across the trust. Your role also involves supporting the production, education, training and dissemination of documentation in relation to medicines management and safety. Working with a wider multidisciplinary team, you will use appropriate skill mix to ensure best practice is employed, monitored and assured for medicines safety.

Pharmacy and Medicines Optimisation at the Trust are well developed and we aim to provide high quality care with an emphasis on services to patients, through a modernised lean process. We are committed to sharing excellence through our teaching role, are clinically orientated, use IT to enable change and host two Specialist Pharmacy Services, a Supra - Regional QC Laboratory, and Regional Procurement Specialists. The department is committed to the use of new technology and we use ASCribe/EMIS, Omnicell electronic storage cabinets, 2 state-of-the-art BD Rowa Vmax robots, and have implemented ChemoCare and TrakCare electronic prescribing systems.

Specific Duties

  1. To promote the safe, effective, appropriate and economic use of medicines as defined by National Statute, Regulatory frameworks or Trust Policies.
  2. To lead in the project management of the nursing aspects of implementation of the medicines optimisation strategy throughout the Trust, particularly preparing new wards or reinforcing procedures on existing wards, and by acting as a resource on Medicines Optimisation for other members of the MDT.
  3. To assist with the production and update of policies and appropriate documentation for any new systems relating to medicines management.
  4. To ensure a high standard of nursing care regarding medicines optimisation is maintained by contributing to revision/creation of guidelines/care pathways and through reflective practice and input to the nurse management agenda.
  5. To create/deliver educational programmes and specialist training for medicines optimisation for all health care staff.
  6. To provide medicines optimisation information on audit, effectiveness, errors, interventions, efficiency, and any other data/information necessary to measure performance.
  7. To share incident learning, national and local, with all health care professionals as appropriate. To monitor departmental responses to national drug alerts and compile reports for the medicines safety committee.
  8. To provide forward audit planning and co-ordination. To complete audits and reports where appropriate e.g. medicines storage arrangements in clinical areas, adherence to Trust policies
  9. Participates in the training of nursing preceptorship students and pre-registration pharmacy graduates and technicians in medicines optimisation.
  10. The post holder will show a continuing commitment to professional development in order to promote evidence-based practice and ensure that personal NMC revalidation requirements are met.
