Advanced Kidney Care Nurse Specialist

Job summary

An exciting opportunity has arisen to join our dynamic, forward-thinking team of specialist Renal nurses. We are looking for a self-confident and motivated person to join us in the provision of a nurse-led service covering clinics and managing caseloads across our Renal units in East and West Kent.

This post is a combination of patient-facingand office-based work requiring excellent clinical, communication and IT skills.

Applicants need experience of Multidisciplinary working and be able to work effectively, both independently as well as within a team environment.

Experience of teaching junior colleagues and patients on an individual and also in a group setting would be very advantageous, as we contribute to a robust in-house education programme and also deliver patient group education sessions.

This post offers regular working hours covering office/clinic hours (approximately 08.00-17.30) Mondays to Fridays.

Main duties of the job

The post holder will work as a member of the Advanced Kidney Care nursing team. Working across East and West Kent to ensure the maintenance of high standards of patient care in response to service needs.o Assess, co-ordinate and deliver appropriate care interventions for a complex group of patients with Chronic Kidney Disease (CKD) incorporating anaemia management, hepatitis B vaccinations, education of patients and their carers (individual/group settings), renal transplant work-up .o Manage own caseload of patients, working closely with Consultant Nephrologists and the Advanced Kidney Care team leaders.o Education and training of renal and non-renal nurseso Assisting in on going and future audit & research projects within the department

Job responsibilities

Deliver person centred care including; planning, assessment, treatment and evaluation of appropriate clinical interventions within a speciality and generic caseload.

Travelling to and running nurse-led clinics in Canterbury Renal Outpatients Department and Satellite Renal Units across East and West Kent.

IV iron administration

Giving IM Hep B vaccinations

Starting patients on home-based anaemia treatments (Erythropoeisis Stimulating Agents and HIF-PH inhibitors)

Delivering ASK consultations - providing clear, concise education & support to patients to facilitate informed decision making regarding their future treatment choices.

Assisting in group educations for patient + carers

Monitoring of bloods & ordering relevant anaemia treatments including screening of prescriptions/ making treatment adjustments for own caseload of patients.

Ensuring Renal Transplant work-up blood tests are up to date who patients are pursuing this treatment option.

Essential

  • Significant post graduate healthcare experience, equivalent to L6 across a range of clinical specialties.
  • Demonstrable knowledge and experience of delivering high quality clinical care using enhanced skills within Kidney care aligned to the four pillars of practice.
  • Excellent written and verbal communication skills
  • Excellent time management and organisational skills
  • Venepuncture & cannulation skills & IV drug administration.
  • IT skills relevant to digital healthcare and role

Desirable

  • Experience of nurse-led clinics

  • Confidence in public speaking

Qualifications and training

  • Current NMC registration
  • Health related degree relevant to professional registration and area of clinical practice or plans to complete degree programme
