Come and work in our growing EKHUFT Therapy team and lead the therapy service in our new Frailty unit at The William Harvey Hospital in Ashford. If you are dynamic, love the pace of same day emergency care and have a passion for Multi-disciplinary team working, then this is the job for you. You will need to provide high quality therapy assessment and care within a same day emergency care culture, ensuring a comprehensive geriatric assessment and holistic approach to our patients.
Main duties of the job
The post focuses on meeting the needs of the Frail population, requiring complex assessment and decision making to provide the right care in the right place with a focused patient centred approach to admission avoidance or short stay (maximum 48hrs). The FAU will operate a direct access principle, enabling frail patients to bypass A&E wherever possible and have immediate access to a dedicated multi-disciplinary frailty team.

- Accountable for the delivery of the therapy service into the Frailty Units at William Harvey Hospital (Ashford)
- Provides visible proactive clinical leadership, acting as the key senior lead for issues, challenges, and escalation in relation to multi-disciplinary care planning, transfer of care and the discharge process
- Accountable for the clinical and operational therapy leadership into the Frailty Unit
- To support the provision of same day emergency care for frailty &/or short stay inpatient care.
- Assess, investigate and provide a comprehensive management plan - utilising the Comprehensive Geriatric Assessment tool and advanced care planning skills
- Be an active member of the specialist frailty outreach service, supporting frail patients within surgical and medical wards.
- To provide timely assessment and discharge to community pathways as appropriate.
- To promote effective communication with patients, carers and other professionals
- To maintain precise and accurate records with audit and research data collection.
- To provide education and advice for staff and develop links with community colleagues
- The post holder will embed the CQC domains into the service and evidence how these are met.
