Role Summary
The role of Community Nursing Operational/Clinical Lead is fundamental to the success of Community Nursing Teams. The post holder will be able to demonstrate the competencies required to perform at this level and role model the Trust's core values as well as exhibit the behaviours required as part of those values.
The Operational/Clinical Lead will have the clinical expertise in Community Nursing to direct, support and develop staff in their clinical roles and will hold the Advance History Taking and Independent Prescribing Qualification.
Promoting a service which prevents patients from having inappropriate or unscheduled hospital admissions and helps facilitate hospital discharge. Implementing evidence-based practice to ensure best patient care.
Works alongside the Senior Nurses to ensure staff are competent in their role and new staff and inducted and supported to reach competency required for their role.
The Operational/Clinical Lead will also be responsible for all aspects of the leadership and development of teams of nurses. They will ensure high quality evidence-based care is delivered to patients, their families and carers in the Community. The Operational/Clinical Lead will be proactive in promoting a positive image of Community Nursing,
Main duties of the job
Ensure care in their locality is based on sound assessment and care planning
Ensure that appropriate Statutory and mandatory training has been undertaken by all team members.
Responsible for the retention, recruitment, induction, personal and professional development of staff.
Monitor clinical supervision, induction, preceptorship and development of staff.
Exercise leadership and judgment in the management of a Community Nursing Team ensuring good clinical practice and maintain high standards of care, review staff practice at regular intervals.
Ensure the service is meeting business and performance requirements of the Organisation in-line with service specification.
Investigate complaints and incidents, including Serious Incidents requiring investigation, safeguarding and clinical issues.
Identify the Root Causes, make recommendations for improvement and disseminate learning to staff, adding areas of concern to the risk register
Strengthen clinical leadership among Band 6 and 7 staff
Collaborate with the learning and development team to build a competency-based workforce that is trained, enabled and supported to deliver best practice care.
When required to work clinically alongside teams to ensure standardisation of practice embedding evidence-based practice.
