Patient Pathway Coordinator- Cancer Services

Job summary

We have an exciting opportunity for an exceptional candidate to join the Epsom & St Helier's Cancer Services administrative team. The successful candidate will support the provision of the highest quality patient care through dedicated administrative and clerical support and continuous improvement. They will be able to demonstrate advanced communication and organisational skills, be able to use their own initiative and show problem solving skills. Excellent attention to detail and a passion for patient care is essential.

Based in Cancer Services Team, the post holder will be the point of contact for all administrative and clerical issues relating to a patient's pathway whilst under the Cancer services care, tracking and booking of appointments. To provide a patient focused service that supports clinical staff in the provision of efficient integrated pathways that enable the best use of resources and ensures that patients are fit, willing and able to be treated and that they are treated in the right place, by the right person, at the right time.

The post holder supports the provision of the highest quality patient care through dedicated administrative and clerical support and continuous improvement.

This post is for Monday-Friday between 08:00am and 17:00pm on a rota basis. Flexible working cannot be considered for this role.

Main duties of the job

To work as part of a clinical team ensuring that suspected cancer patients are booked efficiently for outpatient appointments, diagnostic procedures/ inpatient /day case operations within national guidelines and in accordance with the Trust's Cancer Access Policy.

Ensure that patients are informed with reasonable notice, and that all relevant paperwork is completed for given appointments. This includes ensuring that all elective attendances/admissions are promptly and accurately recorded on the IClip and Electronic Referral System (ERS)

Key Working

To schedule suspected cancer patients in accordance with principles and target waiting times as set out in the NHS Constitution and Health and Social Care Bill 2011 (e.g. 18 Week Referral to Treatment, 62-day and 31-day Cancer Waiting Times, Two-Week Wait standards etc.), expediting patient pathways where necessary. Ensure cancellations and Did Not Attend (DNA) are rescheduled and recorded in accordance with Trust policy.

To provide administrative support to clinical teams including the management of clinician's schedules. Ensure sessions are fully and appropriately utilised.

Verify patient identity and demographic details during every patient encounter to ensure accuracy and currency of information.

Monitor and confirm that referrals are received promptly via the Electronic Referral System (ERS) to maintain timely patient pathway progression. Contact GP practice or relevant teams for any relevant information as required.
