Job summary
As a member of the Stroke team, the post holder will take a key role in leading and developing the Stroke SSNAP requirements across the Stroke Service at Epsom and St Helier Hospital.
The post holder will contribute to the provision of data quality for stroke patients, within the multidisciplinary team, ensuring that high quality administration is delivered within the Trust's Stroke services. In conjunction with the multidisciplinary team, the post holder will record and monitor stroke patient outcomes.
The post holder will provide advice and support to patients, their carers and other health care professionals. The post holder will work within agreed care settings according to established protocols or guidelines, and will contribute to the smooth running of the Service from an inpatient and outpatient setting.
The post-holder will be expected to take a lead role in the collection and analysis of mandatory national Stroke data and the dissemination within the Stroke teams and Medicine Directorate. They will be involved in facilitating Stroke Service Improvement to streamline the patient journey and improve the quality of patient care, and also in developing information systems, in line with Trust strategy.
Main duties of the job
To support the clinical team with timely and accurate data collection and management of the Stroke data for internal and external submission.
Maintain effective working relationships with the wider Stroke team, enabling joint working initiatives between both sites.
