Band 7 Parkinson's disease Practitioner; Community Sutton Health & Care

We have an exciting opportunity in the Sutton Community neuro team for an AHP or nurse who is interested in working with clients who have Parkinson's disease. The post holder will have/ develop a highly specialist knowledge of PD, in particular the medical management of the condition, so a background in neurology is essential. Patients are seen for regular reviews in clinic or their home, so staff need to be able to travel around the borough. Patients also contact the service for advice as required to manage their symptoms.

The role works closely with patients, regional movement disorder centres, neurologists, GPs and other professionals so that patients have their needs identified in a holistic manner, their function is optimised, risks are managed, and they are provided with high quality care. The post holder will have responsibility for a caseload of PD patients & will be expected to work autonomously & flexibly in order to address the needs of those patients, throughout their condition from diagnosis to end of life.

Another aspect of the role is education, both with patients, families, health & social care colleagues, so that patients receive high quality care across pathways. Whilst promoting self-management, safeguard independence & preventing inappropriate hospital stays, maximising rehab &/ or recovery after illness, & minimising premature dependence.

Main duties of the job

  • To act as an autonomous practitioner and be responsible for leading and managing a clinical caseload of patients with Parkinson's disease.
  • To holistically assess patients referred & accepted to the service in a timely manner in order that contractual obligations are met & those patients' needs are effectively identified & appropriate interventions put in place. In line with local & national standards & guidelines.
  • To holistically review patients on the caseload both within clinic & domiciliary visits, in a timely manner, based on need, in order that their associated symptoms, health, social & well-being needs are identified & managed with an emphasis on self-management, in line with local & national standards & guidelines.
  • To advise on changes to mediation & (where qualifications allow) prescribe medication as appropriate, to patients on the caseload within local and national guidelines.
  • To act as an expert resource for patients with PD, working flexibility with a changing demand, in order to meet the changing needs of the patient, including symptoms management, medication management & to avoid unnecessary admissions.

  • Level 1 Registered Nurse or qualified AHP
  • Current NMC or HCPC registration
  • Specialist qualification in Neurological or Parkinson's Disease (PD) nursing, or a commitment to undertake postgraduate training and complete within 18 months
  • Evidence of continued professional development

Desirable

  • A minimum of a year's experience with PD
  • Post graduate training in neuro-disability or Community nursing qualification
  • Palliative care qualification (or experience)
  • Non-medical prescriber qualification
