Job summary

The Head of Transformation and Integration is an essential role within the Sutton Health and Care (SHC) senior leadership structure which enables health and care transformation and integration to happen in co-production with SHC staff, patients and with partner organisations and promotes sustainability across SHC, in association with the 2023-28 Group (Gesh) strategy, Sutton Provider Alliance and, also in association with the PLACE workstreams and expected delivery of the Integrated Neighbourhood Teams (INTs) agenda.

Main duties of the job

This post holder will work in conjunction with the Sutton Provider Alliance principles and strategy in relation to plans to be taken forward yearly with respect of integrated transformation.

The post holder will have lead responsibility for SHC specific health and care developments, transformation and integration in the following areas:

  • INTs - development and collaboration including the proactive care agenda
  • Clinical service review for community nursing for SHC
  • SHC Service Transformation Programmes (internal)
  • Service Transformation Programmes which involve SHC from a SW London, Gesh, PLACE and Provider Alliance perspective (external)
  • Other transformational/integration and collaborative programmes that become a priority for SHC as strategy and planning changes
  • Development of bids completing tenders and awards as appropriate in conjunction with approvals from Gesh/Integrated Care Team meeting
  • Cost Improvement Programme (CIP) to include income generation schemes.

Senior Leadership and Transformational Role

  • To lead on the INT collaboration programme for SHC as part of our key priorities of PLACE. This will involve working with each of the INTs - SHC and social care to collaborate and intertwin services around proactive care and the INT localism agenda
  • To lead on clinical service reviews such as community nursing transformation and planned care and any other internal or external SHC programme work with transforms care and pathways
  • To develop and lead on the programmes of work in year and each year for CIPs, transformation and integration as agreed by Gesh, PLACE, the Provider Alliance and SHC, holding both self to account and managers who are leading on key programmes of work
  • To provide visible and supportive senior leadership and line management responsibility for all staff across areas of the portfolio, as well as supportive leadership for operational managers who are working with the post holder to take programmes of work forward
  • To deputise for the Director of SHC, as appropriate at national and local levels in relation to key areas of the portfolio
  • To chair and/or participate in senior leadership meetings across the place of SW London or Sutton, for instance in relation to the programmes of work as defined by the portfolio
  • Ensure robust financial acumen when developing programmes of work, as well as planning for transactional/transformational aspects of new service delivery working closely with the Deputy Director of SHC and the Finance Team
  • Ensure activity and performance for the service areas in question meets the outcomes and KPIs that SHC are held to account for, ensuring newly developed programmes meet standards as defined by national KPIs or best practice guidance, working closely with the Chief Nurse for Integrated Care and Quality Team when developing such
  • Foster the ideals of co-production when developing and implementing service change. This includes active engagement with patient participation groups, staff and partners
  • As part of the programme of work and in collaboration with partners across Sutton and in co-production with staff ensure that developed programmes have seamless care pathways internal and external to SHC.

Quality, Performance, Risk and Clinical Governance

  • When developing and implementing new programmes of work, continually work to uphold the standards within the services as set out by CQC, regulatory bodies and other legislation, working to the principles that these are business as usual improvements SHC need to make and be assured of
  • When developing and implementing new programmes of work, actively promote good evidence-based practice and ensure that all practice is delivered in accordance with NICE or other guidance, working closely with the Chief Nurse for Integrated Care to enable such
  • Ensure that all clinical services (developed or refreshed) actively promote safe/effective care and provide a positive patient experience as the means of delivering individualised patient care, working closely with patient participation groups to co-produce the re-design
  • For the programmes of work in delivery ensure that there are robust control processes and risk management arrangements aligned to excellent standards of programme management, ensuring full compliance with internal and external governance and best practice requirements
  • Work with the BI Team to actively develop robust performance metrics, dashboards that evidence the delivery of the programme services in development to include performance, productivity and outcome impact. Monitor status of such until these move to a business as usual perspective
  • As part of the programme of work/service delivery identify new skills/core training and development needs for the review of new services in conjunction with the Chief Nurse for Integrated Care
  • Ensure all staff within the Service Improvement Team have annually recorded objectives, appraisals and development plans in line with the Trust Strategy
  • Actively seek out national and local funding, awards and grants as well as awards for areas of best practice/service design, research and development
  • Ensure that complaints and concerns are investigated fully and satisfactorily resolved, working with relevant operational managers to enable this
  • Report monthly to the internal assurance group activity, performance, quality and risk activity for the portfolio

  • Continue to work with SHC and partners to develop services in line with national guidance and in keeping with the SW London, Gesh Strategy
