We are looking for nursing staff from all backgrounds including newly qualified, those more experienced, looking for a change, maybe wanting to work locally, excited by the difference of being out within the community with the benefits and challenges that brings.

To work within the team, contributing to team management and the development of innovative practice. To be responsible for the assessment of care needs and the development, implementation and evaluation of holistic care packages.

We have our own Learning & Development Department that have a range of different courses and development plans that give you the opportunity to grow your career.

Main duties of the job

PCN - Primary Care Network

It is a network formed from GP practice populations working in partnership with others to provide health and care to local communities.

What we do?

  • Provide ongoing care and support
  • Deploy the most appropriate staff member(s) in a coordinated and timely way to best use community resources
  • Reduce risk of admission for patients
  • Support people in their homes, people incare homes and through in-reach in tohospitals where necessary
  • Work closely with the community hospitalsand specialist teams

Our PCN has a community focused trimuvirate (group of three) of Lead GP / Community; Lead Clinician and Operational Manager. Specialist services and community hospitals work closely with the PCNs to ensure a whole system approach to working across Surrey Downs.
