Job summary
This role is focused on providing appropriate, effective interventions and treatments to people with severe and enduring mental health needs, their carers (including friends and relatives) and their supporters in the community, enabling and assisting them to meet the outcomes that are important to them and facilitating engagement with mainstream services.
The post holder will also provide clinical leadership and supervision to team members.
The role involves providing profession specific interventions to people and also bringing professional expertise into the Multi-Disciplinary Team.
In addition, there may be a need to provide non-profession specific, defined, personalised interventions that are agreed through Support Conversations. These might include a range of possible generic, evidence based therapeutic interventions where staff are trained to deliver these interventions.
The role involves being responsible for Personalised Care and Support Planning through the AWP Your Team-Your Conversation-Your Plan approach, including taking on the role of "keyworker" at times andcompleting Safety Assessments. There may also be requirements to support the delivery of Social Care Assessments and reviews where required.
Main duties of the job
- To undertake the full range of activities required to deliver an ongoing comprehensive mental health service.
- To facilitate the delivery of Personalised Care and Support Planning in line with the Your Team your Conversation-Your Plan Policy and guidance.

- Your Conversation - to have regular personalised support conversations with people you are working with. To take dialog outcome measures within the conversations and to use thesestrength based conversations to co-create advance plans to keep people well and safe in the future.
