Job summary

A vacancy has arisen for the team manager post in the Wiltshire Dementia service - overseeing the Care Home liaison and Dementia CIT teams. The teams provide services across Wiltshire and operate from several sites. As a team manager, you will have operational responsibility for the day to day running of Care Home Liaison and dementia CIT teams ensuring the delivery of high-quality clinical assessments and interventions. This role will also involve close/collaborative working with the memory service team manager and senior practitioner roles.

Suitable applicants will need to have substantial experience of working within dementia care and be able to provide clinical leadership to the multidisciplinary team.

- To provide management and leadership forCHL and DCIT.

- To directly manage and supervise staff in the two teams, providing cross-cover for colleagues as necessary.

-To ensure delivery of key targets in relation to service performance, integrated governance, workforce planning, partnership development and financial sustainability.

-To work in close partnership with Service Users, Carers, Primary Care, Trust Performance Management and Finance Staff, Corporate Services, Human Resources, Social and Community Care, Independent and Not for Profit Organisations in the delivery of operational services.
