Job summary

Band 5 CMHT Clinician - Non-Psychotic Pathway

Are you interested in holistic mental health care , keen to work in a community setting and eager to work as part of an Multi professional team? If you can answer yes to these questions then we want you to apply for one of our band 5 posts in Sarum CMHT.

We have redesigned the model of care based on identified pathways and we are looking for clinicians who are skilled or have an interest in training to deliver interventions to service users in the non-psychotic pathway.

We have initiated a training programme of structured clinical management which is evidence based approach for individuals with complex emotional needs who are on a non-psychotic pathway and this training would be available to new staff.

If you are up for the challenge this is an exciting opportunity for clinicians to work in a focused way developing skills for an identified group of individuals.

In addition to this you would also be working with a other groups of people with diagnosis ranging from depression, anxiety and bipolar.

Main duties of the job

Main duties will include:

being a keyworker for individuals , ensuring care is in keeping with the care plan and being delivered by the appropriate professionals.

offer clinical input into care planning.

Risk assessment and mitigation where possible.

Structured Clinical Management.

Group work.

Attending Supervision.

As well as NMC registered nursed this post is also open to newly qualified social workers. In line with AWP trust policy all newly qualified social workers (NQSW) will be recruited initially at Band 5 level, regardless of advertised role banding. All NQSW's are expected to successfully complete their ASYE within 2 years of employment, unless there are exceptional circumstances in which case it may be extended for a further 2 years.

If you have been appointed to an advertised Band 6 role this will be an automatic progression, following confirmation of your successful ASYE completion. If you have been appointed to a Band 5 post you will then be eligible to apply for a subsequent Band 6 vacancy upon successful ASYE completion.

Job responsibilities

Provide a Mental Health service to an allocated caseload of Service Users, and deliver a set of defined interventions within that caseload, having responsibility for the assessment of needs, the planning and implementation of treatment and care to meet those needs as agreed with multidisciplinary team and/or supervisor, key working (with supervision) Service Users, and contributing to the evaluation of care across a variety of settings where Mental Health or Specialist Treatment is available.

Liaise as appropriate with other members of the multidisciplinary team and other agencies involved in the care of individual service users.

Participate in Integrated Governance processes, particularly as they relate to own practice, contributing to the monitoring of service provision and performance, and standards of care, through data collection and audit processes.

This role is focused on providing appropriate, effective interventions and treatments to people with severe and enduring mental health needs, and their carers (including friends and relatives) and their supporters in the community, enabling and assisting them to meet daily health, social care and well-being needs, in line with personal recovery goals, and facilitating engagement with mainstream services.

The focus of the team is to work within the recovery model. You will receive regular supervision and have support from your clinical supervisor as well as the senior practitioner in the team. We cover a wide geographical area and actively promote remote working to reduce travelling times.

Develop and maintain as far as possible effective helping relationships with service users, working in partnership with service users and carers and ensuring that your practice accords with guidance and best practice standards regarding the delivery of services including, but not limited to, requirements in relation to Privacy, Dignity and Respect and Diversity.

Provide a defined Mental Health assessment and interventions to an allocated caseload of service users, having responsibility for the assessment of needs and the planning and implementation of treatment and care to meet those needs, based on evidence of best practice, and as agreed with multidisciplinary team and/or supervisor, and contributing to the evaluation of care.

Working with service users, and others as appropriate, working collaboratively in the ongoing assessment of risk, and the development and review of plans to manage identified risks effectively, working closely with senior members of the multi-disciplinary team and practicing in accordance with Trust and National policy and best practice guidance.

Develop and maintain good working relationships and effective communication with other members of the MDT, participating in regular team meetings and contributing to peer supervision and training as appropriate.

Maintain accurate and up to date records of casework with individual service users, ensuring that your practice and record-keeping complies with all relevant standards and Good Practice Guidelines.

Ensure the monitoring and evaluation of the Service by collecting and recording high quality and timely data using electronic and paper systems as required.
