Job summary
A new role has become available with the Wiltshire Dementia Complex Intervention Team (DCIT).
As a key worker in the team you will be responsible for a caseload of service users with a diagnosis of dementia, working within the Your Team, Your Conversation, Your Plan framework to deliver a range of health and social care interventions. You can expect to receive excellent supervision, and there will be opportunities for career development and training in order to support you in this role.
This post is based in the west of the county and mainly involves working in that area, however as the service is Wiltshire-wide there may be a need, on occasion to travel within the county. Due the size of Wiltshire and the nature of the position, the post holder would need to hold a full driving licence and have access to their own transport for work. It would not be possible to fulfil the requirements of this post using public transport.
Main duties of the job
The Complex Intervention Team aims to provide safe and supportive mental health care for adults with a diagnosis of dementia who are experiencing some level of distress or difficulty due to their illness.
The Team provides a range of community-based health interventions for service users, working in an inclusive way to resolve the immediate difficulties and improve their situation, you will act as a care co-ordinator.
Where appropriate, the post holder will work alongside other services to plan, deliver and review effective interventions to manage risk and prevent hospital admission.
We welcome applicants from band 5 practitioners with relevant post graduate experience, we will also consider band5/6 development post.
