Job summary
A great opportunity has arisen for a Band 4 Recovery Co-ordinator to join the BANES Recovery Service based at Bath NHS House. This is a full time position working Monday to Friday 9-5pm.
You would be joining a friendly, sociable, cohesive, and pro-active team and you would be well supported by those you work with. We are an integrated team and work closely with our Local Authority colleagues, so you would obtain a good working knowledge of both health and social care.
Main duties of the job
The role is focussed on supporting people with severe and enduring mental health needs, and their carers (friends/relatives/carers) and their supporters in the community, enabling and assisting them to meet daily health, social care and well being needs, in line with personal recovery goals, and facilitating engagement with mainstream services.
The post holder will contribute to the ongoing assessment, planning delivery and review of activities and interventions against identified health, social care and well being needs, acting as care/recovery coordinator for an allocated group of service users. The role will also require undertaking and delivering defined activities and interventions for a wider group of identified service users, in accordance with the agreed personal recovery plan. This maybe either on a one to one basis, or as part of a group activity, Some out of hours working maybe required.
