Job summary
An exciting opportunity has arisen for a Band 7 Clinician to join a new Transfer of Care hub, coordinating care for Service Users who are in the most acute phase of a mental health crisis.
The role will involve overseeing a Service User's journey from admission to discharge, ensuring that the flow throughout this critical part of their treatment is seamless and purposeful.
You will join a team that aims to use system knowledge and clinical skills to optimize each patient's outcome and recovery. BNSSG Transfer of Care (TOC) hub is a fast paced and ever evolving environment. You will need to be able to work flexibly and collaboratively both within the TOC team along with internal and external stakeholders.
We are looking for an enthusiastic, highly motivated and dynamic individual who will work closely with the Crisis Teams to review admission requests and ensure Service Users are receiving the right care in the most appropriate setting, for example home treatment or inpatient admission.
Main duties of the job
Day to day responsibilities will include.
Work with inpatient teams to identify service users who are clinically ready and support with problem solving any barriers to discharge.
The post holder will need to be able to bring together stakeholders, with differing viewpoints and priorities, to reach consensus on individual discharge plans.
Lead the co-ordination of service user care in complex cases, discharges or transfer as appropriate, ensuring both health and social care needs are fully assessed.
Recognizing when problems may occur and initiating an escalation to the service manager in a timely manor.
Work with community teams to build community package of care.
Understand barriers to system flow and report themes and current pressures.
Build a working relationship and understanding of roles with in system partners.
Participating in relevant health promotion aspects of the service, including facilitating Inter Agency Meetings and maintaining the Inter Agency Network.
Contribute to the development of policies and procedures within the TOC Hub.
