These new roles will be part of our developing Transfer of Care Hub where you will work with colleagues from local authorities and the voluntary and community and social enterprise sector.
The Hub aims to ensure we provide people with the right care within the community so that they aren't in hospital any longer than is necessary.
Please note: This post is open to newly qualified social workers. In line with AWP trust policy all newly qualified social workers (NQSW) will be recruited initially at Band 5 level, regardless of advertised role banding. All NQSW's are expected to successfully complete their ASYE within 2 years of employment, unless there are exceptional circumstances in which case it may be extended for a further 2 years.
If you have been appointed to an advertised Band 6 role this will be an automatic progression, following confirmation of your successful ASYE completion. If you have been appointed to a Band 5 post you will then be eligible to apply for a subsequent Band 6 vacancy upon successful ASYE completion.
Main duties of the job
As a Clinical Case Reviewer, you will play a crucial role in ensuring the quality and effectiveness of our mental health services. Your responsibilities will include:
- Influencing and leading on all aspects of discharge planning
- Providing expert advice to multidisciplinary teams
- Liaising between commissioners and providers
- Understanding and monitoring how the Mental Health Act in particular Section 117 aftercare arrangements are implemented
- Agreeing new plans of care
- Reviewing and where necessary leading on changes to existing packages of care
- Identifying areas for development to enhance patient care.
- Participating in audits and quality improvement initiatives.
- Providing training and support to clinical staff on case review processes and standards.
- Contributing to the development of clinical guidelines, policies and procedures
- Registered healthcare professional (e.g., Nurse, Social Worker, Occupational Therapist)
- Extensive experience in mental health services
- Strong analytical and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Commitment to continuous improvement and professional development.
